Approve Rules of registration and issue of the certificate on belonging to citizenship of Ukraine which are attached.

Rules of registration and issue of the certificate on belonging to citizenship of Ukraine

1. The certificate on belonging to citizenship of Ukraine (daleesvidetelstvo) is the document which confirms citizenship of Ukraine of person with age up to 16 years.

2. The certificate is drawn up:

to children under 7 years - for a period of up to 3 years;

to children the senior 7 years - before achievement of 16 years.

3. The certificate is drawn up with use of the State information system of registration accounting of physical persons and their documentation.

4. Documents acceptance and issue of the certificate is performed by territorial authorities or divisions of GMS (further - territorial authorities (divisions)).

Production of certificates is performed by the state company "The state center of personalisation of documents.

5. For execution of the certificate by one of parents or other legal representative of the child (further - the applicant) in the relevant territorial authority (division) in the place of residence of the child move:

the statement questionnaire on the sample established by the Ministry of Internal Affairs;

certificate of birth of the child;

the reference which confirms citizenship of Ukraine;

two photographs in size 3, x 4,5 of centimeter (for children age 7 years are more senior).

Documents are accepted on condition of presentation by the applicant of the passport of the citizen of Ukraine, in case of the address of person to which the status of the refugee in Ukraine is provided, - the certificate of the refugee, in case of the address of the foreigner or the stateless person - the national passport or the document which replaces it.

6. The official of territorial authority (division) checks the submitted documents and in case of compliance to their established requirements certifies the statement questionnaire.

In case of need the applicant is offered to offer further explanations or to make amendments.

7. Preparation of the electronic version of the statement questionnaire (with photoreflection) for receipt of the certificate is performed by the employee of territorial authority (division) on sample according to appendix.

8. The basis for registration and issue of the certificate is the decision made by the chief of the relevant territorial authority (division) about what in the statement questionnaire the corresponding entry is made.

The statement questionnaire remains in the relevant territorial authority (division).

9. To the certificate the following data are entered:

surname, name and middle name;

nationality with indication of the finding bases;

birth date;

birthplace;

personal number, except the cases provided by the law;

floor;

date of issue and effective period.

All entries in the certificate are performed in Ukrainian.

10. The certificate is granted to the applicant not later than in 30 days after adoption of documents.

About issue of the certificate in the statement questionnaire the corresponding mark becomes, and the fact of its obtaining makes sure the signature of the applicant.

11. In case of damage or loss of the certificate the applicant shall notify on it the relevant territorial authority or division in the place of residence of the child. Repeated execution of the certificate is performed according to the procedure, determined by these Rules.

12. In case of the termination of effective period of the certificate or achievement of 16-year age by the child the certificate is given in the relevant territorial authority (division) in the place of residence of the child.

13. Destruction of certificates is performed according to the procedure, the established Ministry of Internal Affairs.

14. Actions and divergence of officials who skirt procedure and deadlines of registration and issue of the certificate can be appealed in the supreme territorial authority (division) or in court.
