1. This Instruction establishes procedure for technical investigation of the reasons of the accidents and incidents which took place on hazardous production facilities and (or) potentially dangerous objects and also accounting treatment for such accidents and incidents.
2. In this Instruction terms and their determinations in the values established by the Law of the Republic of Belarus of January 5, 2016 "About industrial safety" are applied (The national legal Internet portal of the Republic of Belarus, 14.01. 2016, 2/2352).
3. Technical investigation of the reasons of accidents on hazardous production facilities and (or) potentially dangerous objects is conducted by the special commission which structure is created according to parts of the fourth or sixth article 34 of the Law of the Republic of Belarus "About industrial safety".
4. When conducting technical investigation of causes of accident of the special commission the right is granted:
receive explanations, including written, conduct survey of the workers of the subject of industrial safety concerning accident and (or) being witnesses of accident and also other officials of the subject of industrial safety;
request and study necessary documents regarding fulfillment of requirements of industrial safety which can be related to the happened accident.
The documents submitted to the special commission shall not contain erasures and corrections.
5. The special commission when conducting technical investigation of the reasons of accidents:
carries out studying of technical condition of objects of industrial safety to accident and establishes compliance of technical and operational characteristics and technical condition of objects of industrial safety to the requirements established by rules on ensuring industrial safety;
makes survey and fixing of on-scene situation of accident by means of technical and other means, including photo, video filming with execution of the protocol of survey of the place of accident;
establishes circumstances and causes of accident, the extent of damage suffered, persons which allowed violations of requirements of industrial safety;
develops actions for elimination of causes of accident and their prevention further;
determines the number of copies of materials by technical investigation of causes of accident.
6. If necessary the chairman of the special commission organizes carrying out technical calculations, laboratory researches, testing, examinations and other actions with involvement of specialists from the scientific and design organizations.
