About approval of the Procedure and forms of submission of data on specimen signatures, prints of seals and/or stamps

According to Item 3 of the resolution of the Cabinet of Ministers of Ukraine of November 4, 2015 I order to No. 890 "Some questions of ensuring implementation of electronic service for putting down of apostille":

1. Approve enclosed:

1) Procedure for submission of data on specimen signatures, prints of seals and/or stamps;

2) Form of submission of data on specimen signatures, prints of seals and/or stamps;

3) Form of submission of data on modification of data on specimen signatures, prints of seals and/or stamps.

2. To department of state registration (Benedisyuk S. V.), to Department concerning notariate (Zusik T. V.) provide representation of the Electronic register of apostilles of data to the administrator about:

specimen signatures, prints of seals and/or stamps of the officials of the Ministry of Justice of Ukraine authorized on putting down of apostille and also officials of departments of state registration of acts of civil status, the state registrars who performed state registration of legal entities and physical persons entrepreneurs, notaries who signed and put down seal and/or stamp on official documents to which operation of the Convention canceling the requirement of legalization of foreign official documents in day of introduction of the electronic register of apostilles extends;

modification of data on specimen signatures, prints of seals and/or stamps of the officials of the Ministry of Justice of Ukraine authorized on putting down of apostille within three working days from the date of approach of corresponding changes.

3. To provide to chiefs of the head territorial administrations of justice in areas and the city of Kiev representation of the Electronic register of apostilles of data to the administrator about:

specimen signatures, prints of seals and/or stamps of officials of departments of state registration of acts of civil status, the state registrars performing state registration of legal entities and physical persons entrepreneurs, notaries;

modification of data on specimen signatures, prints of seals and/or stamps of officials of departments of state registration of acts of civil status, the state registrars performing state registration of legal entities and physical persons entrepreneurs, notaries within three working days from the date of approach of corresponding changes.

4. To provide to the administrator of the Electronic register of apostilles filling of this register with data on specimen signatures, prints of the seals and/or stamps which arrived according to Procedure for submission of data on specimen signatures, prints of seals and/or stamps, the Form of submission of data on specimen signatures, prints of seals and/or stamps and the Form of submission of data on modification of data on specimen signatures, prints of the seals and/or stamps approved by this order.

5. To department of the civil, financial legislation and legislation concerning land relations () submit A.N. Ferens this order on state registration according to the Presidential decree of Ukraine of October 3, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".

6. This order becomes effective from the date of its official publication.

Procedure for provision of data on specimen signatures, prints of seals and/or stamps

1. This Procedure determines conditions, the bases and the procedure of submission of data on specimen signatures, prints of the seals and/or stamps necessary for filling of the electronic register of apostilles (further - the register), and also data on changes in such data.

2. Data on specimen signatures, prints of seals and/or stamps, and also data on changes in such data move the subjects authorized according to the legislation in certain forms.

Form of submission of data on specimen signatures, prints of seals and/or stamps of officials of departments of state registration of acts of civil status, the state registrars performing state registration of legal entities and physical persons entrepreneurs, state and private notaries, and also form of submission of data on changes in such data are certified by the signature of the head of territorial authority of the Ministry of Justice of Ukraine and are sealed by official.

Form of submission of data on specimen signatures, prints of seals and/or stamps of judges, and also form of submission of data on changes in such data are certified by the signature of the chief of staff of court and vessels are sealed official.

Form of submission of data on specimen signatures, prints of seals and/or stamps of officials of archive organizations, the Public archive agency of Ukraine, and also form of submission of data on changes in such data make sure the signature of the head of the Public archive agency of Ukraine and are sealed by official.
