1. Determine that in case of realization of pilot project on state registration of the scrap entered by the order of the Cabinet of Ministers of Ukraine of July 13, 2016 No. 502-r "About introduction of realization of pilot project on state registration of scrap" (further - the Pilot project), departments of state registration of acts of civil status which participate in its realization in appropriate places:

1) conduct separate register of statements for state registration of scrap and the book of accounting of forms of marriage certficates with compliance with law on record keeping in departments of state registration of acts of civil status;

2) provide immediate processing of the request for carrying out state registration of scrap (appendix 1) and the service provision agreement on the organization of carrying out state registration of the scrap signed between applicants and the organizer of state registration of scrap according to the recommended approximate agreement (appendix 2);
