According to Item 226.3 of Article 226 of the Section VI of the Tax code of Ukraine, the resolution of the Cabinet of Ministers of Ukraine of December 27, 2010 to No. 1251 "About approval of the Regulations on production, storage, sale of brands of the excise tax and marking of alcoholic beverages and tobacco products", to the subitem 5 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375, I order:

1. Approve enclosed:

1) the Preliminary request calculation about the need for brands of the excise tax;

2) Summary request calculation for production of necessary quantity of brands of the excise tax;

3) the Request calculation for acquisition of brands of the excise tax for marking of alcoholic beverages;

4) the Request calculation for acquisition of brands of the excise tax for marking of tobacco products;

5) Report on use of brands of the excise tax;

6) Magazine of registration of the importer (customer) of alcoholic beverages, tobacco products;

7) the Register of issue of brands of the excise tax for marking of alcoholic beverages of national production;

8) Register of issue of brands of the excise tax for marking of alcoholic beverages of import production;

9) the Register of issue of brands of the excise tax for marking of tobacco products of national production;

10) the Register of issue of brands of the excise tax for marking of tobacco products of import production;

11) the Register of the returned brands of the excise tax;

12) the Report on movement and remaining balance of brands of the excise tax for marking of alcoholic beverages and tobacco products;

13) Deeds register, confirming introduction of payment for brands of the excise tax.

2. Declare invalid the order of the Ministry of the income and charges of Ukraine of February 4, 2014 No. 115 "About approval of forms of requests calculations for production and acquisition of brands of the excise tax, the report on use of brands of the excise tax and magazines for accounting of brands of the excise tax", registered in the Ministry of Justice of Ukraine on February 25, 2014 for No. 329/25106.

3. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Romaniuk Yu. P.) to provide in accordance with the established procedure:

provision of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. This order becomes effective from the date of its official publication.

5. To impose control over the implementation of this order on the Deputy Minister of Finance of Ukraine Makeeva A. L. and Chairman of the Public fiscal service of Ukraine Nasirov R. M.
