For the purpose of implementation of the order of the Ministry of Health of the Kyrgyz Republic of 05.10.2011 "About enhancement of system of maintenance and repair of medical equipment in the organizations of health care of the Kyrgyz Republic for 2011-2015" and streamlinings of organization activity of health care of the Kyrgyz Republic on utilization of written off medical equipment I order to No. 488:

1. Approve the enclosed instruction "About procedure for utilization of written off medical equipment in the organizations of health care of the Kyrgyz Republic".

2. To heads of the organizations of health care of the Kyrgyz Republic to carry out utilization of written off medical equipment according to the instruction "About procedure for utilization of written off medical equipment in the organizations of health care of the Kyrgyz Republic", approved by Item 1 of this order.

3. To impose control of execution of this order on the deputy minister Kaliyev M. T.

The instruction "About procedure for utilization of written off medical equipment in the organizations of health care of the Kyrgyz Republic"

Chapter 1. General part

1.1. This instruction determines procedure for utilization of written off medical equipment in the organizations of health care of the Kyrgyz Republic.

1.2. The organizations of health care of the Kyrgyz Republic shall provide timely utilization of written off medical equipment, and also observe established procedure of creation and provision of the reporting on the taken measures.

1.3. Head of the organization of health care:

- bears the personal responsibility for the organization of sending for utilization of written off medical equipment to the legal or individual entrepreneurs performing their procurement, conversion and realization from among having according to the legislation of the Kyrgyz Republic of the license for the specified types of activity;

- appoints the order on the organization of health care of responsible persons which carry out work on delivery on utilization of written off medical equipment on processing enterprises in the quantity allowing to provide its completeness and timeliness and exercises control of the organization of work on utilization of written off medical equipment on processing enterprise.

1.4. Responsibility for conducting financial accounting of utilization of written off medical equipment is born by the head of financial division of the organization of health care (chief accountant).

1.5. Persons guilty of violation of established procedure of utilization of written off medical equipment bear responsibility according to the legislation of the Kyrgyz Republic.

1.6. Collection, accounting, storage and delivery on utilization of the written off medical equipment containing precious metals and film photox-ray materials are carried out according to Chapter 7 of this instruction.

1.7. The organizations of health care represent twice a year (as of July 1 and on January 1 the current year) to superior organizations the reporting on utilization of written off medical equipment (appendix 1 and appendix 2).

Chapter 2. Collection of written off medical equipment in the organizations of health care of the Kyrgyz Republic

2.1. The organizations of health care carry out collection of the medical equipment which is written off in accordance with the established procedure, the equipment, aggregates, nodes, details and other metal products of household purpose for utilization on processing enterprise on permanent basis.

2.2. Written off medical equipment is reflected in the acts of write-off which are drawn up according to "The rules on write-off of the medical equipment in the organizations of health care of the Kyrgyz Republic" approved by the order MZ KR of 06.07.2011 No. 341.

2.3. The medical equipment which became useless or lost consumer properties manufactured of non-ferrous and (or) ferrous metals and their alloys or having as a part of part from non-ferrous and (or) ferrous metals and their alloys is subject in utilization according to this instruction.

2.4. Availability of scrap and waste of the non-ferrous and (or) ferrous metals received from liquidation, dismantling or other disposal of medical equipment is reflected in acts of write-off of fixed assets.

2.5. Scrap and the waste of non-ferrous and (or) ferrous metals received from dismantling or dismantle of medical equipment are credited based on the act of receipt of the material values received during the dismantling and dismantle of buildings and constructions according to No. M-35 form (appendix 3).

2.6. Scrap and the waste of non-ferrous and (or) ferrous metals which is formed following the results of production processes in the organizations of health care are accepted on warehouse of this organization for the write-off order according to No. M-11 form (appendix 4).

2.7. Acceptance of scrap and waste of non-ferrous and (or) ferrous metals on warehouse of the organization is made by selection committee as a part of at least three employees appointed by the head of the organization of health care.

Chapter 3. Accounting of scrap and waste of non-ferrous and (or) ferrous metals in the organizations of health care of the Kyrgyz Republic

3.1. The basis for acceptance of scrap and waste of non-ferrous and (or) ferrous metals on warehouse, is the write-off order constituted according to form the No. M-11 and the act of write-off of fixed assets approved by the head of OZ and sealed official.

3.2. The write-off order, the act of write-off of fixed assets are registered the employee (responsible person) appointed by the order on the organization of health care responsible for collection, accounting, storage and delivery of scrap and waste of non-ferrous and (or) ferrous metals to processing enterprises in register of receipt (delivery) of scrap and waste of non-ferrous and (or) ferrous metals (appendix 5).

3.3. Scrap and waste of non-ferrous and (or) ferrous metals are warehoused by each type separately and designated by label with sequence number of the log entry of accounting of receipt of scrap and waste of non-ferrous and (or) ferrous metals.

3.4. Collection of scrap and waste of non-ferrous and (or) ferrous metals is performed on specially allocated platforms.

Chapter 4. Storage of scrap and waste of non-ferrous and (or) ferrous metals in the organizations of health care of the Kyrgyz Republic

4.1. Storage of scrap and waste of non-ferrous and ferrous metals is performed according to safety regulations and fire-proof requirements.

4.2. Warehousing on specially allocated platforms of scrap and waste of non-ferrous and (or) ferrous metals shall be performed separately from each other and ensure safety of their quantity and quality, possibility of free survey and loading of raw materials, and also the organization of accounting and possibility of conducting checks.

4.3. Collected scrap and waste of non-ferrous and (or) ferrous metals shall not mix up, become soiled the garbage and impurity preventing melting.

4.4. In storage locations of scrap and waste of non-ferrous and (or) ferrous metals there shall be board of documentation on which take place:

- the statement from the order of the head of the organization of health care on the admission of persons to works in warehouse;

- safety rule and to measures of fire safety;

- samples of accounting and credit and debit documents and form of their filling;

- inventory of the internal equipment, stock and tools.

Chapter 5. Delivery of scrap and waste of non-ferrous and (or) ferrous metals on processing enterprises

5.1. Alienation of scrap and waste of non-ferrous and (or) ferrous metals is performed based on the purchase and sale agreement signed between the organization of health care and processing enterprise.
