Based on subitem 9.1 of Item 9 of the Regulations on the Ministry of Health of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of October 28, 2011 No. 1446 "About some questions of the Ministry of Health and measures for implementation of the Presidential decree of the Republic of Belarus of August 11, 2011 No. 360" the Ministry of Health of the Republic of Belarus DECIDES:

1. Approve the enclosed Instruction about procedure for ensuring disability with forms of leaves.

2. This resolution becomes effective since February 1, 2016.

The instruction about procedure for ensuring disability with forms of leaves

1. This Instruction establishes procedure for ensuring disability of the organizations of health care with forms of leaves, including the departmental, other organizations performing medical activities, specified in Item 7.5 of the list of the ministerial procedures performed by state bodies and other organizations for petitions from citizens, No. 200 approved by the Presidential decree of the Republic of Belarus of April 26, 2010 "About the ministerial procedures performed by state bodies and other organizations for petitions from citizens" (The national register of legal acts of the Republic of Belarus, 2010, No. 119, 1/11590) (further if other is not established, - the organizations of health care).

2. For the purposes of this Instruction the main terms and their determinations in the values established by the Law of the Republic of Belarus of June 18, 1993 "About health care" are used (Vyarkho¸naga Savet's Vedamastsi of Respubliki Belarus, 1993, No. 24, Art. 290; National register of legal acts of the Republic of Belarus, 2008, No. 159, 2/1460).

3. Providing the organizations of health care with forms of leaves of disability, their storage and accounting are performed by the production and trade republican unitary enterprise "Belmedtekhnika" (further - Belmedtekhnika Unitary Enterprise).

4. Issue of forms of leaves of disability Belmedtekhnika Unitary Enterprise is made according to the plan of distribution approved by the Ministry of Health of the Republic of Belarus:

to head departments, managements (departments) of health care regional, Healthcare Committee Minsk city executive committees (further - managements of health care) or to the organizations authorized by them;

to the state organizations of health care subordinated to the Ministry of Health of the Republic of Belarus.

5. Managements of health care of orders of heads determine procedure for issue of forms of leaves of disability to the state organizations of health care, to other organizations performing medical activities, located in the territory of the respective area (Minsk).

6. Issue of forms of leaves of disability to the organizations of health care is performed:

Belmedtekhnika Unitary Enterprise, managements of health care or the organizations authorized by them to the state organizations of health care on a grant basis;

managements of health care or the organizations authorized by them to other organizations on paid basis for a fee in the amount of costs for their acquisition.

7. The organizations of health care provide, storage, accounting and use of forms of leaves of disability according to the procedure, established by the legislation.
