About approval of standards of the state services in the field of health care

(as amended on 27-09-2019)

According to the subitem 1) article 10 of the Law of the Republic of Kazakhstan of April 15, 2013 "About the state services" and the subitem 6) Item 1 of article 7 of the Code of the Republic of Kazakhstan of September 18, 2009 "About health of the people and health care system" PRIKAZYVAYU:

1. Approve:

1) the standard of the state service "Challenge of the Doctor on the House" according to appendix 1 to this order;

2) the standard of the state service "Making an appointment with the doctor" according to appendix 2 to this order;

3) the standard of the state service "Attachment to the Medical Organization Giving Primary Health Care" according to appendix 3 to this order;

4) the standard of the state service "Voluntary Anonymous and Obligatory Confidential Physical Examination on HIV Infection Availability" according to appendix 4 to this order;

5) the standard of the state service "Issue of the Reference from the Antitubercular Organization" according to appendix 5 to this order;

6) the standard of the state service "Issue of the Reference from the Psychoneurological Organization" according to appendix 6 to this order;

7) the standard of the state service "Issue of the Reference from the Narcological Organization" according to appendix 7 to this order;

8) the standard of the state service "Issue of the Statement from the Medical Record of the Inpatient" according to appendix 8 to this order;

9) the standard of the state service "Issue of the Reference from the Medical Organization Giving Primary Health Care" according to appendix 9 to this order;

10) the standard of the state service "Issue of Leaf about Temporary Disability" according to appendix 10 to this order;

11) the standard of the state service "Issue of the Certificate of Temporary Disability" according to appendix 11 to this order;

12) the standard of the state service "Registration of Consent or Withdrawal of Consent to Lifetime Donation of Fabrics (Part of Fabric) and (or) Bodies (Part of Bodies) after Death for the purpose of Transplantation" according to appendix 12 to this order;

13) the standard of the state service "Passing of Preliminary Obligatory Medical Examinations" according to appendix 13 to this order;

14) the standard of the state service "Issue of Information on the Transferred Amounts of Assignments and (or) Fees on Compulsory Social Medical Insurance" according to appendix 14 to this order;

15) the standard of the state service "Issue of Certificates of Participation in System of Compulsory Social Medical Insurance as the Consumer of Medical Services" according to appendix 15 to this order;

16) the standard of the state service "Rendering Emergency Medical Service" according to appendix 16 to this order;

17) the standard of the state service "Issue of the Direction to Patients on Hospitalization in Hospital within the Guaranteed Amount of Free Medical Care through the Portal of Bureau of Hospitalization" according to appendix 17 to this order;

18) the standard of the state service "Issue of the Certificate of the Admission to Vehicle Control" according to appendix 18 to this order;

19) the standard of the state service "Acceptance and Consideration of Documents on Possibility of the Direction of Citizens of the Republic of Kazakhstan for Treatment Abroad at the expense of Budgetary Funds" according to appendix 19 to this order;

20) the standard of the state service "Issue of the Conclusion about Needs in Sanatorium Treatment" according to appendix 20 to this order.

2. To provide to department of the organization of medical care of the Ministry of health and social development of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) the direction on official publication of this order within ten calendar days after its state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction on official publication in periodic printing editions and information system of law of Ad_let;

3) publication of this order on Internet resource of the Ministry of health and social development of the Republic of Kazakhstan.

3. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Tsoi A. V.

4. This order becomes effective after ten calendar days after day of its first official publication.

to the Order of the Minister of health and social development of the Republic of Kazakhstan of April 27, 2015 No. 272

Standard of the state service "Challenge of the Doctor on the House"

Chapter 1. General provisions

1. The state service "Challenge of the Doctor on the House" (further – the state service).

2. The standard of the state service is developed by the Ministry of Health of the Republic of Kazakhstan (further - the Ministry).

3. The state service appears the medical organizations giving primary health care (further – the service provider).

Reception of applications and issue of results of rendering the state service is performed through:

1) the service provider (in case of the direct address or on telephone communication of uslugopoluchatel);

2) the web portal of "the electronic government" www.egov.kz (further - the portal).

Chapter 2. Procedure for rendering the state service

4. Term of rendering the state service:

in case of the address to the service provider (it is direct or on telephone communication):

1) from the moment of delivery by uslugopoluchatel of documents to the service provider – no more than 10 (ten) minutes;

2) the most allowed time of expectation for delivery of documents – 10 (ten) minutes;

3) the most allowed time of servicing of uslugopoluchatel – 10 (ten) minutes;

in case of the address through the portal:

from the moment of delivery by uslugopoluchatel of documents to the service provider – no more than 30 (thirty) minutes.

The state service in case of the direct address or by phone to the service provider appears in day of the address. At the same time the request for receipt of the state service is accepted in 2 hours prior to completion of work of the service provider (till 18:00 o'clock in the working days, till 12:00 on Saturday).

The state service is rendered in electronic format in day of the address on the portal. At the same time the request for rendering the state service is accepted in 2 hours prior to completion of work of the service provider (till 18:00 o'clock in the working days, till 12:00 o'clock on Saturday).

5. Form of rendering the state service: electronic (partially automated) and (or) paper.

6. Result of rendering the state service:

1) in case of the direct address or on telephone communication – the log entry of registration of challenges of the service provider and the oral answer with indication of date, time of visit of the doctor;

2) in electronic format in case of the address on the portal – the notification in the form of the status of the electronic request in private office.

Thus after acceptance of request for rendering the state service to uslugopoluchatel at the scheduled time medical care is provided at home.

7. The state service is rendered free of charge.

8. Working schedule:

1) the service provider – Monday through Saturday (Monday – Friday from 8:00 till 20:00 o'clock without interruption, on Saturday from 9:00 till 14:00 o'clock), except days off (Sunday) and holidays according to the Labor code of the Republic of Kazakhstan;

2) the portal – round the clock, except for the technical breaks connected with carrying out repair work (in case of the address of uslugopoluchatel after the termination of working hours, in days off and holidays according to the Labor code of the Republic of Kazakhstan reception of applications and issue of results of rendering the state service is performed the next working afternoon).

9. Documents necessary for rendering the state service in case of the address of uslugopoluchatel (or his representative by proxy):

1) to the service provider:

the identity document, for identification in case of the direct address;

2) on the portal:

request in electronic form.

Data on identity documents, the service provider receives from the corresponding state information systems through lock of "the electronic government".

9-1. The basis for refusal in rendering the state service is:

1) establishment of unauthenticity of the documents submitted by uslugopoluchatel for receipt of the state service, and (or) these (data) containing in them;

2) lack of attachment to this medical organization giving primary health care according to the order of the Minister of health and social development of the Republic of Kazakhstan of April 28, 2015 to No. 281 "About approval of Rules of rendering primary health care and Rules of attachment of citizens to the organizations of primary health care".

Chapter 3. Procedure for appeal of decisions, actions (failure to act) of the service provider and (or) his officials concerning rendering the state service

10. Decisions, actions (failure to act) of the service provider and (or) his officials concerning rendering the state services are appealed by submission of the claim addressed to the head of the service provider or the Ministry of the address specified in Item 12 of this standard of the state service or to the address: 010000, Mr. Nur-Sultan, Prospekt Mang_l_k El 8, House of the Ministries, entrance No. 5.

Confirmation of adoption of the claim is its registration (stamp, the entering number and date) in office of the service provider or the Ministry with indication of surname and initials of person which accepted the claim, term and the place of receipt of the answer to the made complaint. After registration the claim goes to the head of the service provider or the Ministry for determination of the executive in charge and acceptance of adequate measures.

The claim of uslugopoluchatel which arrived to the service provider is subject to consideration within five working days from the date of its registration. The motivated answer about results of consideration of the claim goes to uslugopoluchatel by mail or is issued purposely in office of the service provider. In case of the address through the portal the uslugopoluchatel obtains information on procedure for appeal by means of single contact center concerning rendering the state service by phone: 8-800-080-7777, 1414.

When sending the electronic address through the portal information on the address which is updated during processing of the address by the service provider is available to uslugopoluchatel from "private office" (mark about delivery, registration, execution, the answer about consideration of the claim).

In case of disagreement with results of the rendered state service the uslugopoluchatel addresses with the claim to authorized body by assessment and control of quality of rendering the state services.

The claim of uslugopoluchatel which arrived to authorized body by assessment and control of quality of rendering the state services is considered within fifteen working days from the date of its registration.

11. In disagreement cases with results of the rendered state service, the uslugopoluchatel takes a legal action in the procedure established by the legislation of the Republic of Kazakhstan.

Chapter 4. Other requirements taking into account features of rendering the state service including rendered electronically

12. Addresses of places of rendering the state service are placed on Internet resource of the Ministry www.dsm.gov.kz, the Section "State Services".

13. Uslugopoluchatel has possibility of receipt of the state service electronically through the portal.

14. The service provider has possibility of receipt of information on procedure and the status of rendering the state service in the mode of remote access by means of portal "private office", and also single contact center concerning rendering the state service.

15. Contact telephone numbers of service desks concerning rendering the state service are specified on Internet resource of the Ministry www.dsm.gov.kz. Single contact center concerning rendering the state services: 8-800-080-7777, 1414.
