About some questions of acquisition, transportation, storage, leave, use and destruction of drugs, psychotropic substances and precursors in healthcare institutions

In pursuance of Item 2 of the resolution of the Cabinet of Ministers of Ukraine of May 13, 2013 to No. 333 "About approval of the Procedure for acquisition, transportation, storage, leave, use and destruction of drugs, psychotropic substances and precursors in healthcare institutions", according to Item 8 of the Regulations on the Ministry of Health of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 267, I order:

1. Approve enclosed:

1) form of primary accounting documentation No. 129-6/u "Register of medicines of drugs, psychotropic substances and precursors in departments and offices of treatment and prevention facilities of health care" and Instruction for its filling;

2) form of primary accounting documentation No. 129-7/u "Register of medicines of drugs, psychotropic substances and precursors on posts of departments of treatment and prevention facilities of health care" and Instruction for its filling;

3) form of primary accounting documentation No. 129-8/u "Register of medicines of drugs, psychotropic substances and precursors in pharmaceutical (pharmaceutical) organizations" and Instruction for its filling;

4) form of primary accounting documentation No. 129-9/u "Register of medicines of drugs, psychotropic substances and precursors in treatment and prevention facilities of health care for accomplishment of appointments of the doctor in stationary conditions" and the Instruction for its filling;

5) form of primary accounting documentation No. 129-10/u "Register of the created hospitals returned from at home medicines of drugs, psychotropic substances and precursors" and the Instruction for its filling;

8) regulations of calculation of amounts of storage of medicines of drugs, psychotropic substances and precursors in general in treatment and prevention facility of health care, and also by its departments, posts, offices;

9) form of primary accounting documentation No. 129-12/o "The magazine of registration of special prescription forms of form No. 3 (f-3) in healthcare institutions" and the Instruction for its filling.

2. Approve Changes in the order of the Ministry of Health of Ukraine of July 19, 2005 No. 360 "About approval of Rules of writing out of recipes and requirements orders of medicines and products of medical appointment, the Procedure for leave of medicines and products of medical appointment from drugstores and their structural divisions, the Instruction about procedure for storage, accounting and destruction of prescription forms and requirements orders", registered in the Ministry of Justice of Ukraine on July 20, 2005 for No. 782/11062, which are applied.
