According to subitem 7-1) of article 8 of the Law of the Republic of Kazakhstan "About gaming", the subitem 2) of Item 3 of article 16 of the Law of the Republic of Kazakhstan "About the state statistics" I ORDER:

1. Approve:

1) form of submission of the reporting of the organizer of gaming, according to appendix 1 to this order;

2) Rules of implementation of collection and the analysis of the reporting represented by the organizer of gaming according to appendix 2 to this order.

2. To committee on cases of sport and physical culture of the Ministry of Culture and sport of the Republic of Kazakhstan (Kanagatov I. B.) in the procedure established by the legislation:

1) to direct this order to state registration in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration in the Ministry of Justice of the Republic of Kazakhstan to provide official publication of this order in periodic printing editions and information system of law of Ad_let;

3) after official publication to place this order on Internet resource of the Ministry of Culture and sport of the Republic of Kazakhstan;

To provide 4) to Department of legal service of the Ministry of Culture and sport of the Republic of Kazakhstan data on execution of the actions provided by this Item within ten working days from the date of execution of actions.

3. To impose control of execution of this order on the supervising vice-Minister for Culture and Sports of the Republic of Kazakhstan.

4. This order becomes effective after ten calendar days after day of its first official publication.

Chapter 1. General provisions

1. This explanation determines single requirements for filling of form of submission of the reporting of the organizer of gaming.

2. The form is filled with organizers of gaming (legal entities, performing the organization and carrying out gamblings and (or) bet), and it is represented in authorized body in the field of gaming.

Rules of implementation of collection and the analysis of the reporting represented by the organizer of gaming

Chapter 1. General provisions

1. These rules of implementation of collection and the analysis of the reporting represented by the organizer of gaming (further – Rules), are developed according to the Law of the Republic of Kazakhstan "About gaming" and determine procedure for collection and the analysis of the reporting represented by organizers of gaming.

2. The purpose of collection and the analysis of the reporting is implementation of monitoring of activities of subjects of gaming on compliance to the legislation on gaming.

Chapter 2. Procedure of collection of the reporting

3. Collection of the reporting is performed by means of provision by organizers of gaming in authorized body in the field of gaming (further - authorized body) the reporting under form, according to appendix 1 to this order (further - the reporting).

4. The reporting is presented in the paper or electronic form quarterly no later than the 10th following the accounting period.

5. The reporting contains the following information:

1) data about business identification number of the organizer of gaming;

2) name of the organizer of gaming;

3) type of activity;

4) legal address;

5) data on the number of workers of the organizer of gaming for the beginning of the accounting period;

6) data on the number of workers of the organizer of gaming by the end of the reporting period;

7) the information about the number of participants of gamblings and (or) bet (residents) for the beginning of the accounting period;

8) the information about the number of participants of gamblings and (or) bet (nonresidents) for the beginning of the accounting period;

9) information about the number of participants of gamblings and (or) bet (residents) at the end of the period;

10) information about the number of participants of gamblings and (or) bet (nonresidents) at the end of the period;

25) data on change of quantity of tables, replacements of the equipment, including serial or accession number of the replaced equipment;

26) data on change of quantity of tables, replacements of the equipment, including about quantity of the game tables specified in case of receipt of the license;

27) data on change of quantity of tables, replacements of the equipment, including information on increase and reducing quantity of game tables;

28) the information about beneficial owners as a part of founders, heads of the organizer of gaming and about the first heads of the organizer of gaming.

6. The reporting is certified by the signature of the head of the organizer of gaming, or his face replacing (with application of a copy of the order on assignment of obligations), and also seal of the organization (in the presence) and goes to authorized body.

7. The provided reporting is registered in authorized body and is considered by its relevant structural division.

Chapter 3. Procedure of the analysis of the reporting

8. The authorized body based on the provided reporting performs the analysis on availability and completeness of all necessary data.

The analysis of the provided reporting includes:

1) consideration regarding compliance to the qualification requirements imposed to organizers of gaming;

2) forming of data on not provision or not complete provision of the reporting.

9. Analysis results are used by preparation of suggestions for improvement of state policy in the field of gaming and carrying out preventive control without visit of subject (object) of control.

10. The authorized body by collection, generalization and the analysis of the reporting creates data on observance of qualification requirements by organizers of gaming.
