About procedure for creation and the direction in law-enforcement bodies of the Republic of Belarus of the act of burial of unclaimed bodies (remains) of the dead and unknown corpses

1. Establish form of the act of burial of unclaimed bodies (remains) of the dead and unknown corpses (further - the act of burial) according to appendix 1 to this resolution.

2. Determine that:

2.1. statements of burial are drawn up by the specialized organizations *, performed burial of unclaimed body (remains) of the dead or unknown corpse;

2.2. statements of burial are not drawn up in case of burial (reburial) of bodies (remains) of the dead in the years of war found on places of fights, unaccounted military burials and places of death of defenders of the Fatherland and the victims of wars;

2.3. no later than five working days after burial of unclaimed body (remains) of the dead or unknown corpse the specialized organizations provide creation in duplicate of the act of burial with assignment to it the corresponding number in register of acts of burial according to appendix 2 to this resolution.

Maintaining in the specialized organization of register of acts of burial in electronic form on condition of ensuring safety of the data containing in it is allowed;

2.4. one copy of the act of burial together with the copy of the certificate of death or the medical death certificate (still birth) no later than the working day following behind day of creation of the act of burial goes the specialized organization to information center of Main Department of Internal Affairs of the Minsk city executive committee, the Department of Internal Affairs of regional executive committee for territoriality according to appendix 3 to this resolution.

The second copy of the act of burial is located with the specialized organization in separate case which is stored according to rules of clerical work.

3. This resolution becomes effective after its official publication.
