3. Extend the form of the state statistical reporting specified in Item 1 of this resolution on the legal entities (except the Instructions listed in part three of Item 1 on filling of form of the state statistical reporting 1-is sensible "The report of the organization rendering medical services to the population", approved by this resolution) having special permission (license) for implementation of medical activities, their separate divisions having separate balance which render medical services to the population.
4. Recognize invalid:
