2. To provide to department of the organization of medical care of the Ministry of health and social development of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction on official publication in periodic printing editions is also information - system of law of Ad_let;

3) placement of this order on the Internet - resource of the Ministry of health and social development of the Republic of Kazakhstan.

3. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Tsoi A. V.

1. Subject of the agreement

1. The subject of health care within the guaranteed amount of free medical care (further - GOBMP):

- provides to the Patient medical care (pre-medical medical care, qualified medical care, specialized medical care, highly specialized medical care, the medico-public assistance, including specialized and highly specialized medical care with the round-the-clock medical observation, hospital-replacing, rehabilitation and palliative), according to the legislation of the Republic of Kazakhstan.

- provides timely purpose of medicamentous therapy and writing out to the patient of recipes on receipt of free medicines and products of medical appointment at the out-patient level according to the List of medicines and products of medical appointment for free providing the population within the guaranteed amount of free medical care at the out-patient level with certain diseases (conditions) and specialized medical products, the deputy Minister of health care of the Republic of Kazakhstan approved by the order of November 4, 2011 No. 786 (further - the Order No. 786).

2. The patient implements the recommendations of the expert Subjekt of health care about observance of the mode set by the doctor including connected with acceptance of the written-out medicines and products of medical appointment, providing high-quality provision of medical services.

3. The relations between the Subject of health care and the Patient are regulated by regulations of the legislation of the Republic of Kazakhstan (further - RK) and this agreement.

4. The patient agrees to inclusion and use of the personal data (surname, name, middle name (in case of its availability), nationality, floor, birth date, individual identification number, legal address, the residence, subscriber number of the means of communication) and medical records in information systems of the Subject of health care and the Ministry of health and social development of the Republic of Kazakhstan (further - MZSR RK) for the purpose of exchange of this information of participants of process of inspection and treatment of the Patient.

2. Conditions and procedure for rendering services

The subject of health care renders services under this agreement to the address: ____________, and also in the medical organizations having with the Subject of health care the relevant agreements of sub contracting for rendering medical services within the guaranteed amount of free medical care (further - GOBMP).

7. The subject of health care renders services under this agreement in days and business hours which are established by administration of the Subject of health care and are brought to the attention of the Patient.

8. The subject of health care within GOBMP renders to the Patient medical services (pre-medical medical care, qualified medical care, specialized medical care, highly specialized medical care, the medico-public assistance, including specialized and highly specialized medical care with the round-the-clock medical observation, hospital-replacing, rehabilitation and palliative), according to the legislation of the Republic of Kazakhstan.

9. In case of planned hospitalization of the patient in hospital in reception inspection by the doctor of reception, no later than 30 minutes is performed from the moment of receipt.
