1. Approve enclosed:

- the list of the documents necessary for decision making about payment of insurance sum to the military personnel and persons liable for call-up called on training and special sessions insured on compulsory national insurance of life and health, and to persons equated to them;

- the list of mutilations (injuries, wounds, contusions) relating to heavy or easy in the presence of which the decision on loss occurrence at insured on compulsory national insurance of life and health of the military personnel and persons liable for call-up called on training and special sessions, and equated to them to persons is made.

2. Recognize invalid:

- The order of the Government of the Kyrgyz Republic of September 4, 2000 No. 544 "About approval of the Regulations on compulsory national insurance of life and health of the military personnel of Armed Forces of the Kyrgyz Republic and the persons liable for call-up called on training and special sessions";

The list of the documents necessary for decision making about payment of insurance sum to the military personnel and persons liable for call-up called on training and special sessions insured on compulsory national insurance of life and health, and to persons equated to them

1. In case of death (death) of insured person during passing of military service, military charges are drawn up:

- the statement for payment of insurance sum from each beneficiary on compulsory national insurance (minor children of insured person join in the statement of one of spouses, the guardian or the custodian) with appendix of personal bank details of each beneficiary for transfer of money and the copy of the identity document of each beneficiary;

- the certificate of the commander of military unit (the chief of organization, the head of the organization) of circumstances of loss occurrence and the size of official pay rate and salary on military rank on the date of loss occurrence of insured person;

- copy of the death certificate of insured person;

- the copy of the statement from the order of the commander of military unit (the chief of organization, the head of the organization) on exception of insured person of lists of staff of military unit (organization, the organization);

- copies of the documents confirming family relation of beneficiaries with insured person;

- the copy of the resolution of guardianship and custody body on establishment of guardianship or custody of insured person over the ward;

- the copy of the document of the guardianship and custody body confirming absence of parents at insured person and the fact of his education and content by beneficiaries;

- the certificate of educational institution of training of children of insured person aged from 18 up to 23 years with indication of date of transfer on training;

- the copy of the reference confirming the disability establishment fact to children of insured person before achievement of 18-year age by them, issued by medico-social commission of experts;

- copy of the certificate on the right to inheritance;

- the declaration of abandonment of the beneficiary of receipt of insurance sums on compulsory national insurance of life and health, the right to which he has according to the laws others, than the Law of the Kyrgyz Republic "About compulsory national insurance of life and health of the military personnel and persons liable for call-up called on training and special sessions, and persons equated to them". - the statement for payment of insurance sum from each beneficiary on compulsory national insurance (minor children of insured person join in the statement of one of spouses, the guardian or the custodian) with appendix of personal bank details of each beneficiary for transfer of money and the copy of the identity document of each beneficiary;
