1. Approve the enclosed Rules of issue, accounting and maintaining personal medical books.

2. To provide to committee on consumer protection of the Ministry of national economy of the Republic of Kazakhstan in the procedure established by the legislation:

1) the state regitsration of this order in the Ministries of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order its direction on official publication in periodic printing editions and in information system of law of Ad_let;

3) placement of this order on official Internet resource of the Ministry of national economy of the Republic of Kazakhstan.

3. To impose control of execution of this order on the supervising vice-minister of national economy of the Republic of Kazakhstan.

4. This order becomes effective after ten calendar days from the date of its first official publication.

Rules of issue, accounting and maintaining personal medical books

1. General provisions

1. These rules of issue, accounting and maintaining personal medical books (further - Rules) are developed according to Item 10 of article 155 of the Code of the Republic of Kazakhstan "About health of the people and health care system".

2. Rules determine single procedure for issue, accounting and maintaining personal medical books.

3. In these rules the following concepts are used:

1) the personal medical book - the personal document issued to the representative of dekretirovanny national group in which results of obligatory medical examinations with mark about the admission to work are entered;

2) dekretirovanny national group - the persons working in services industry, constituting the greatest danger to infection of people around with infectious and parasitic diseases.

4. Acquisition of the personal medical book is performed by the representative of dekretirovanny national group at the physical persons and legal entities implementing them in form according to appendix 1 to these rules.

2. Procedure for issue, accounting and maintaining the personal medical book

5. The responsible specialist of the subject of health care (further - the specialist) issues the personal medical book to the representative of dekretirovanny national group by results of medical examination and laboratory researches.

6. Accounting of the personal medical book is made in register of the personal medical book of subjects of health care where medical examination in form according to appendix 2 to these rules is performed.

7. The personal medical book of faces of dekretirovanny group is stored on workplace.

8. The personal medical book is filled in with the specialist in the state or Russian languages.

9. The photo of the owner is enshrined in the personal medical book by seal of the subject of health care where medical examination is performed.

10. "Passport data" are made to Section 1 of the personal medical book data of the owner of the personal medical book on the identity certificate or the passport, its personal signature, series and number of the personal medical book.

11. Data on profession and position, place of employment, birth date, the residence of the owner of the personal medical book are made to Section 2 of the personal medical book of "The data on the owner of the personal medical book".

12. To Section 3 of the personal medical book "The postponed infectious diseases" the specialist makes earlier postponed infectious diseases.

13. In Section 4 of the personal medical book "Result of medical examination, the therapist" is brought the conclusion of the doctor by results of medical examination of the owner of the personal medical book which is fixed by its personal signature and seal with indication of surname, name and middle name (in the presence) (further - First name, middle initial, last name) the doctor.

14. "Result of inspection on tuberculosis" make result of radiological inspection of the owner of the personal medical book with investment of picture and the conclusion of the doctor of the medical organization or the physical person having the license for occupation private medical practice according to the procedure and on conditions, stipulated by the legislation the Republic of Kazakhstan in the field of the health care performing medical examination which is fixed by its personal signature with indication of First name, middle initial, last name and seal to Section 5 of the personal medical book.

15. "The result of medical examination and laboratory research on venereal diseases" are brought in Section 6 of the personal medical book results of laboratory researches of the owner of the personal medical book and the conclusion of the doctor of the state (private) medical organization or the physical person practising private medicine, performing medical examination, issued according to the procedure and on conditions, stipulated by the legislation the Republic of Kazakhstan in the field of health care, fastened with the personal signature of the doctor with indication of its surname, name and middle name and seal.

 15-1. Data on the carried-out vaccination of the owner of the personal medical book are made to Section 7 of the personal medical book of "Data on preventive inoculations".
