Based on parts one and the second Item 7, of part five of Item 10 and Item 14 of the Regulations on procedure for issue of permissions to special water use, introduction of changes in them and (or) amendments, prolongation of term, the termination of their action and issue of duplicates approved by the resolution of Council of Ministers of the Republic of Belarus of March 2, 2015 No. 152, of Item 9 of the Regulations on the Ministry of Natural Resources and Environmental Protection of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of June 20, 2013 No. 503, the Ministry of Natural Resources and Environmental Protection of the Republic of Belarus DECIDES:
1. Establish forms:
1.1. statements for issue of permission to special water use (for legal entities and individual entrepreneurs) according to appendix 1;
1.2. statements for receipt of the conclusion about possibility of production declared by the water user (for legal entities and individual entrepreneurs) amounts of underground waters according to appendix 2;
1.3. the conclusions about possibility of production of the amounts of underground waters declared by the water user according to appendix 3;
1.4. register of permissions to special water use according to appendix 4;
1.5. statements for issue of permission to special water use (for citizens) according to appendix 5;
1.6. statements for receipt of the conclusion about possibility of production declared by the water user (for citizens) amounts of underground waters according to appendix 6.
2. This resolution becomes effective since May 21, 2015.
