For medical activities

1.

for legal entities and physical persons - rooms or buildings on the property right or the lease agreement or the trust management of state-owned property conforming to standards of the organization of delivery of health care of profile services for the declared subspecies of medical activities

data on compliance to the qualification requirements imposed when licensing medical activities in form according to appendix 1 to these qualification requirements

Acquires data on the documents certifying the property right the service provider from the State database "Register of the Real Estate" integrated with the state E-licensing database, except for lease agreements or the trust management of state-owned property concluded for the term of less than one year which provide the electronic copy of the document

2.

for legal entities and physical persons - the medical and (or) special equipment, the equipment and tools, devices, furniture, stock, transport and other means (if necessary), according to the declared subspecies of medical activities

data on compliance to the qualification requirements imposed when licensing medical activities in form according to appendix 1 to these qualification requirements

3.

for physical persons - the corresponding education according to the declared subspecies of medical activities

data on compliance to the qualification requirements imposed when licensing medical activities in form according to appendix 1 to these qualification requirements

Information about the highest (data since 2015) or secondary medical education, the service provider receives from information system of the Ministry of Education and Science of the Republic of Kazakhstan, except for persons which ended till 2015, and also persons which ended outside the territory of the Republic of Kazakhstan who provide the copy of the education document

4.

for physical persons - specialization or enhancement and other types of advanced training over the last 5 years on the declared subspecies of medical activities

data on compliance to the qualification requirements imposed when licensing medical activities in form according to appendix 1 to these qualification requirements

5.

for physical persons - the appropriate certificate of the specialist, except for specialists of sanitary and epidemiologic profile

data on compliance to the qualification requirements imposed when licensing medical activities in form according to appendix 1 to these qualification requirements

The service provider receives data on the certificate of the specialist from the state E-licensing database, except for persons who received till 2014 who provide the copy of the certificate of the specialist
