According to the subitem 3) of Item 23 of Rules of use of target current transfers from the republican budget for 2015 regional budgets, budgets of the cities of Astana and Almaty on rendering social protection and the help to the population approved by the order of the Government of the Republic of Kazakhstan of February 19, 2015 No. 82, I ORDER:

1. Approve:

1) form of the social contract of activization of family, according to appendix 1 to this order;

2) form of the individual plan of the help to family, according to appendix 2 to this order.

2. Declare invalid the order of the Minister of Labour and Social Protection of the population of February 25, 2014 No. 52-O "About approval of form of the social contract of activization of family" (No. 200 registered in the Register of state registration of regulatory legal acts for No. 9195 and published on October 14, 2014 (27821) in the Kazakhstanskaya Pravda newspaper, on October 14, 2014 No. 200 (28423) in the Egemen Kazakstan newspaper).

3. To provide to department of the public assistance of the Ministry of health and social development of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction on official publication in periodic printing editions and information system of law of Ad_let;

3) placement of this order on Internet resource of the Ministry of health and social development of the Republic of Kazakhstan;

4) finishing this order to regional, Astana and Almaty of managements of coordination of employment and social programs.

4. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Zhakupova S. K.

5. This order becomes effective after ten calendar days after day of its first official publication and extends to the relations which arose since January 1, 2015.
