About approval of the list of necessary data for the passport of production of ethyl alcohol and alcoholic products

1. Approve the enclosed list of necessary data for the passport of production of ethyl alcohol and alcoholic products.
2. To committee of state revenues of the Ministry of Finance of the Republic of Kazakhstan (Ergozhin of E.) in the procedure established by the legislation to provide:
1) the state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;
2) within ten calendar days after the state registration of this order its direction on official publication in periodic printing editions and information system of law of Ad_let;
3) placement of this order on Internet resource of the Ministry of Finance of the Republic of Kazakhstan.
3. This order becomes effective after ten calendar days after day of its first official publication.

1. The passport of production of ethyl alcohol and alcoholic products (further - the passport) developed and approved by producer of ethyl alcohol and alcoholic products (further - the producer) and signed by the head, or person authorized by it, contain the following data:
1) name of the producer (legal entity or individual entrepreneur);
2) number and date of issue of the certificate * or certificates of the state registration (re-registration) of the legal entity, the name of the body which granted the certificate or the reference;
3) legal address of the producer;
4) data of the head of the producer (in the presence at the producer of the production manager to reflect its data (surname, name, middle name (in case of its availability), office phone number and the fax);
5) place of implementation of activities of the producer (production location, postal address, e-mail address (in the presence);
6) IIN - individual identification number / BIN - business and identification number;
7) the name of the serviced bank, the settlement account, the individual identification code (IIC);
8) pattern of ownership;
9) state registration of the rights to real estate;
10) railway details of the producer;
11) year of input of production in operation, when carrying out reconstruction (date);
12) the master plan of production with explication of buildings and constructions (in the form of appendix to the passport);

15) types of the issued alcohol;
16) range of products;
17) flow sheet of production of ethyl alcohol and alcoholic products;
18) the list of the capital processing equipment on workshops and sites in case of production of ethyl alcohol and alcoholic products;
19) about alcohol cellar (the opened or closed type);
20) on technical and chemical production control of ethyl alcohol and alcoholic products;
21) about consumption of the main raw materials and auxiliary materials in case of production of ethyl alcohol and alcoholic products;
22) about storage facilities in case of production of ethyl alcohol and alcoholic products;
23) about metrological support of production of ethyl alcohol and alcoholic products;
