About approval of the Nation-wide target social program of counteraction to HIV infection/AIDS for 2014-2018

The Verkhovna Rada of Ukraine decides:

1. Approve the Nation-wide target social program of counteraction to HIV infection/AIDS for 2014-2018 which is attached.

2. This Law becomes effective from the date of, its publication following behind day.

The nation-wide target social program of counteraction to HIV infection/AIDS for 2014-2018

Program purpose

The purpose of the Program is decrease in the illness rate and death rate from HIV infection/AIDS, provision of high-quality and available services in prevention and diagnosis of HIV infection, first of all to representatives of groups of the increased risk of infection of HIV, services in treatment, medical care, leaving and support of the people living with HIV within reforming of health care system.

Questions of counteraction to HIV infection/AIDS is one of the priority directions of state policy in health sector and social development and subject of the international obligations of Ukraine in the sphere of counteraction to HIV infection/AIDS, in particular on goal achievement of development, proclaimed in the Millennium Declaration of the United Nations, and accomplishment of the Political declaration of 2011 of the United Nations on activization of efforts for eradication of HIV infection/AIDS.

Today the epidemic situation is characterized by the high level of spread of HIV infection among representatives of different national groups, first of all persons belonging to group of the increased risk of infection of HIV, preferential damage of faces of working-age, uneven spread and change of the main way of transfer of HIV with parenteral on sexual. Epidemic of HIV infection is factor of negative impact on able-bodied population of Ukraine and leads to negative social and economic consequences.

From registration of the first case of HIV infection in 1987 till January 1, 2013 in the country 223 530 cases of HIV infection (from them 36830 children up to 14 years), including 56382 persons with disease AIDS are registered (from them 1224 children up to 14 years). The number of the people living with HIV and being under observation in healthcare institutions makes more than 129 thousand people, indicator of prevalence of HIV infection - 283,6 of the person on 100 thousand population, at more than 24 thousand people living with HIV, the disease reached final stage - AIDS. The indicator of prevalence of AIDS constitutes 52,9 of the person on 100 thousand population. For the period observations from 1987 to 2012 of 28498 people died of the diseases caused by AIDS. According to estimative data, at the beginning of 2012 230 thousand people aged from 15 years living with HIV lived in Ukraine make percent 0,58 from total quantity of the population of the specified age category.

At the same time as a result of accomplishment of the Nation-wide program of ensuring prevention of HIV infection, treatment, leaving and support of HIV-positive people and patients with AIDS for 2009-2013 the tendency to stabilization of epidemic situation is observed, in particular the indicator of rates of surplus of new cases of HIV infection decreased (with percent 16,8 in 2006 to percent 3,6 in 2011), the number of cases of HIV infection in age group from 15 to 24 years (since 2775 in 2005 till 1647 in 2012), decreased almost six times (with percent 27,8 in 2004 to percent 4,7 in 2009) the indicator of transmission frequency of HIV from mother to the child decreased.

Despite significant progress, quantity of the registered HIV infection cases annually increases, at the same time the access level to services in treatment, especially among representatives of groups of the increased risk of infection there is HIV, is low and consequently, the mortality rate from AIDS remains high.

Ways and methods of the problem resolution

Three ways of solving the problem are possible.

The first option provides integrated approach to counteraction of epidemic which was applied within accomplishment of the Nation-wide program of ensuring prevention of HIV infection, treatment, leaving and support of HIV-positive people and patients with AIDS for 2009-2013. However such approach requires enhancement taking into account the direction of development of epidemic process within the last five years in Ukraine and the world.

The second option provides strategy implementation of the UN on overcoming epidemic of HIV infection in the world for 2011-2015. However considering difficult economic and social situation and carrying out reforming of health care system in regions of goal achievement of the specified Strategy by 2015 in full (reduction twice quantities of cases of transfer of HIV infection sexually, and also reduction of maternal death rate owing to HIV/AIDS, quantities of death from tuberculosis among HIV-positive people; prevention of transfer of HIV infection from mother to the child, prevention of new cases of spread of HIV infection among the persons consuming drugs) is impossible.

The third, optimal variant provides creation of system of continuous provision of high-quality and available services in prevention and diagnosis of HIV infection, first of all to representatives of groups of the increased risk of infection of HIV, services in treatment, leaving and support of the people living with HIV within reforming of health care system in the way:

optimizations of system of provision of medical care and social services, ensuring professional training of personnel (family doctors, employees of the organizations and institutions providing services to representatives of groups of the increased risk of infection of HIV and their partners, people who live with HIV);

observance of the rights of the people living with HIV;

ensuring access of the population to services in consultation, testing for HIV infection and performing its diagnostics;

observance of the rights of health workers to healthy and safe working conditions;

providing within accomplishment of the Program of priority of scope treatment, leaving and support of the people living with HIV, and their environment;

increases in efficiency of preventive actions concerning representatives of groups of the increased risk of infection of HIV for the purpose of decrease in rates of spread of HIV infection;

forming of the tolerant attitude of the population towards the people living with HIV for the purpose of overcoming their discrimination;

applications genderno the oriented approach when planning and implementing actions in the sphere of counteraction to HIV infection/AIDS;

ensuring interaction central and local executive bodies in case of realization of state policy in the sphere of counteraction to HIV infection/AIDS;

involvement of public associations to rendering to representatives groups of the increased risk of infection of HIV and the people living with HIV, services in prevention, treatment, leaving and support;

development and implementation of actions for prolongation of accomplishment of effective programs of counteraction to HIV infection/AIDS, including those which are carried out at the expense of charitable fees;

introductions of permanent social dialogue in the sphere of employment relationships between executive bodies, employers and labor unions concerning the people living with HIV and patients with AIDS (prevention of discrimination, forming of the tolerant attitude towards the people living with HIV and observance of their rights, safe concerning infection of HIV of working conditions).

Forecast amounts and sources of financing of the Program are given in appendix 1.

Tasks and actions

The list of tasks and actions for accomplishment of the Program are given in appendix 2.

The expected results, efficiency of the Program

Accomplishment of the Program will allow:

it is considerable to reduce number of new cases of infection of HIV among the population with gradual prevention of infection;

reduce by 50 percent risk of infection of HIV of health workers in case of delivery of health care to patients;

reduce by 50 percent quantity of new cases of infection of HIV among representatives of groups of the increased risk;

reduce to 1 percent quantity of cases of infection as a result of transfer of HIV infection from mother to the child;

provide access for representatives of groups of the increased risk of infection of HIV to participation in preventive programs;

cover 100 percent of pupils and students during accomplishment of programs of prevention of HIV infection/AIDS and forming of healthy lifestyle;

provide the people living with HIV and being under observation in healthcare institutions, medical care and social services in leaving and support;

provide access to continuous treatment with medicines of anti-retrovirus therapy of the people living with HIV and patients with AIDS who need it;

reduce by 50 percent death rate from tuberculosis among patients with the combined infection HIV/tuberculosis;

involve in permanent participation in programs of replacement therapy at least 35 percent of consumers of opioid injecting drugs which need it, and also to participation in programs of their rehabilitation;

create at the population the tolerant attitude towards the people living with HIV and representatives of groups of the increased risk concerning infection of HIV and to reduce the level of their discrimination by 50 percent.

The expected results of accomplishment of the Program are given in appendix 3.

Amounts and sources of financing

Financing of the Program is planned to be performed at the expense of means state and local budgets, Global fund for fight against AIDS, tuberculosis and malaria, and also other sources.

The amount of financing of the Program at the expense of means of the government budget is determined annually proceeding from specific objectives and opportunities of financial provision in the corresponding budget period.
