I order:

1. Establish:

Requirements to the state of health of separate categories of citizens (appendix No. 1 to this order);

Amounts of the obligatory diagnostic testings conducted prior to survey of the military personnel of the Armed Forces of the Russian Federation, the citizens undergoing military charges through the Armed Forces of the Russian Federation, for the purpose of, not specified in Item 52 of the Regulations on military-medical examination approved by the order of the Government of the Russian Federation of July 4, 2013 No. 565 (appendix No. 2 to this order);

The list of the additional obligatory diagnostic testings conducted prior to survey of the military personnel of the Armed Forces of the Russian Federation, the citizens undergoing military charges through the Armed Forces of the Russian Federation for the purpose of determination of category of the validity to military service (except for surveys for the purpose of provision of medical leave or release from fulfillment of duties of military service) (appendix No. 3 to this order).

2. Determine Structure of the military-medical commissions performing survey of the military personnel of the Armed Forces of the Russian Federation, the citizens undergoing military charges through the Armed Forces of the Russian Federation, for the purpose of, specified in the paragraph the second Item 51 of the Regulations on military-medical examination approved by the order of the Government of the Russian Federation of July 4, 2013 No. 565 (appendix No. 4 to this order).

Requirements to the state of health of separate categories of citizens

I. Requirements to the state of health of the citizens enlisting under the contract in the Armed Forces of the Russian Federation and the citizens passing military service under the contract in the Armed Forces of the Russian Federation for determination of the validity to passing of military service in certain areas and areas

1. The military-medical commissions take out the conclusion about the validity of the citizens enlisting under the contract in the Armed Forces of the Russian Federation (further - the citizens enlisting under the contract), and the citizens passing military service under the contract in the Armed Forces of the Russian Federation (further - the citizens passing military service under the contract), to passing of military service in the region of the Far North and areas equated to them <*> in the absence of the following mutilations at them, the diseases interfering passing of military service in the specified areas and areas:

mutilations, diseases on which in the III graph of categories of the validity to military service of the Section II "Schedule of diseases" of Requirements to the state of health of citizens in case of initial statement on military accounting, conscription (military charges), the citizens enlisting under the contract, the citizens arriving in military schools, the military personnel, citizens staying in inventory of the Armed Forces of the Russian Federation (appendix to the Regulations on military-medical examination approved by the order of the Government of the Russian Federation of July 4, 2013 No. 565) (further - the schedule of diseases), the limited validity to military service is provided;

mutilations, diseases on which individual assessment of category of the validity to military service is provided in the III column of categories of the validity to military service of the schedule of diseases (except for mutilations, diseases provided by Articles of 33-, 34-b, 67-b, 68-b, 69-b, 73-b, 75-b, 76-b, 77-b, 84-b, the 87th schedules of diseases);

acute disease or exacerbation of chronic disease;

mental and behavioural disturbances (except lungs short-term organic, including symptomatic, the easy and moderately expressed, short-term neurotic, somatoformny frustration and frustration connected with stress), epilepsies with frequent attacks or the epileptic status in the anamnesis or mental disturbances;

the heavy or progressing organic diseases of the central nervous system (vascular damages of brain with resistant focal symptomatology, multiple sclerosis, side amyotrophic sclerosis, cerebral palsy, siringomiyeliya, myopathy, myasthenia, consequences of the postponed tubercular meningitis), and also diseases and injuries of peripheral nervous system with moderate and considerable dysfunction of extremity;

diseases of the haematogenic system with considerable changes in composition of blood and periodic aggravations;

active forms of tuberculosis of any localization, and also inactive tuberculosis in the next three years after disappearance of signs of activity;

the obliterating trombangiit (disease of the Burgher), the obliterating atherosclerosis of vessels of the lower extremities;

chronic recidivous inflammatory diseases of female genitals with moderate and considerable malfunction;

the chronic progressing diseases of the ENT organs requiring the systematic treatment and observation of the otorhinolaryngologist proceeding with frequent aggravations;

chronic diseases of the respiratory organs which are often becoming aggravated with respiratory (pulmonary) insufficiency of II or III degrees;

bronchial asthma of heavy degree or moderate severity;

blood circulatory system diseases with heart failure II, III or IV functional classes, and also tension stenocardias II, III or IV functional classes;

hypertension with considerable dysfunction of "target organs";

the resistant, not giving in to treatment forms of violations of rhythm and conductivity of heart (total atrioventricular block, atrioventricular blockade of the II degree with Morganyi-Adams-Stokes's syndrome, paroksizmalny ventricular takhiaritmiya, ventricular premature ventricular contraction of the III-V gradation on B.Lown, syndrome of weakness of sinusovy node);

rheumatism, Bekhterev's disease, rheumatoid arthritis, diffusion diseases of connecting fabric;

the chronic recuring and progressing diseases of the digestive system (chronic hepatitis, pancreatitis, the stomach ulcer of stomach and duodenum with frequent recurrence and also complicated by bleeding, perforation, penetration or stenosis of the gatekeeper, nonspecific ulcer colitis, disease Krone, severe forms of enterit, the complicated forms of gastroezofagealny reflux disease);

generalized periodontal disease and periodontosis of heavy degree;

chronic diseases of kidneys (with 2 on 5 stages) with frequent recurrence and (or) nephrotic syndrome;

malignant new growths irrespective of stage and results of treatment;

the benign tumors of any localization inclined to rapid growth or leading to malfunction of body;

glaucomas, the progressing diseases of optic nerve, pigmentary degeneration of retina, lowering of visual acuity to 0,03 on both eyes without correction;

chronic widespread diseases of skin, except for common and total forms of gnezdny alopetion and vitiligo.

2. The military-medical commissions take out the conclusion about the validity of the citizens enlisting under the contract and the citizens passing military service under the contract to passing of military service in mountain areas (1500 meters and more above sea level) in the absence of the following mutilations at them, the diseases interfering passing of military service in the specified areas:

vascular diseases of brain and spinal cord with passing violations of brain blood circulation;

diseases of endocrine system with malfunction;

diseases of the haematogenic system with moderate malfunction and rare aggravations;

chronic diseases of respiratory organs with respiratory insufficiency;

blood circulatory system diseases with heart failure II, III or IV functional classes, tension stenocardias II, III or IV functional classes;

paroksizmalny supraventricular takhiaritmiya, Wolf's syndrome - Parkinson - Whyte, permanent atrioventricular blockade of the II degree without syndrome of Morganyi - Adams - Stokes, the developed total block of legs of bunch of Gis;

chronic diseases of kidneys (with 2 on 5 stages) with frequent recurrence and (or) nephrotic syndrome;

malignant new growths irrespective of stage and results of treatment;

the obliterating trombangiit (disease of the Burgher), the obliterating atherosclerosis of vessels of the lower extremities;

acute disease or exacerbation of chronic disease;

mental and behavioural disturbances (except lungs short-term organic, including symptomatic, the easy and moderately expressed, short-term neurotic, somatoformny frustration and frustration connected with stress);

epilepsies with frequent attacks or the epileptic status in the anamnesis or mental disturbances;

the organic diseases of the central nervous system which are followed by hydrocephaly, paralyzes, paresis, speech disturbance, sight;

tumors of brain or spinal cord and other heavy and quickly progressing diseases of the central nervous system with permanent malfunction of bodies;

blood diseases in the presence of progressing signs;

active forms of tuberculosis of any localization, and also inactive tuberculosis in the next three years after disappearance of signs of activity;

hypertension with moderate and considerable dysfunction of "target organs";

generalized periodontal disease and periodontosis of heavy degree;

glaucomas, the progressing diseases of optic nerve, pigmentary degeneration of retina, lowering of visual acuity to 0,03 on both eyes without correction;

the benign tumors of any localization inclined to rapid growth or leading to malfunction of body.

3. The military-medical commissions take out the conclusion about the validity of the citizens enlisting under the contract, and the citizens passing military service under the contract, to passing of military service in areas with adverse climatic conditions in the Republic of Buryatia, the Republic of Tyva, Zabaykalsky Krai which did not enter the list of the region of the Far North and the areas equated to them <*>, places of dislocation of military forming of the Russian Federation in the Republic of Tajikistan, and also in the territories of the Republic of Kazakhstan relating earlier to the city of Leninsk of the Kyzyl-Orda region with the territory which was under administrative supervision of Lenin city council of People's Deputies, to the Dzhezkazgan region, the Mugodzhar district of the Aktyubinsk region in the absence of the following mutilations at them, the diseases interfering passing of military service in the specified areas (places of dislocation) in the specified territories:

chronic diseases of respiratory organs with respiratory (pulmonary) insufficiency of II or III degrees;

bronchial asthma of heavy degree;

recidivous rheumatism (two and more attacks within one year);

chronic nephrite of any form regardless of nature of its current;

the obliterating trombangiit (disease of the Burgher), the obliterating atherosclerosis of vessels of the lower extremities;

the chronic progressing diseases of the ENT organs requiring the systematic treatment and observation of the otorhinolaryngologist proceeding with frequent aggravations;

chronic widespread diseases of skin, except for common and total forms of gnezdny alopetion and vitiligo;

mental and behavioural disturbances (except lungs short-term organic, including symptomatic, the easy and moderately expressed, short-term neurotic, somatoformny frustration and frustration connected with stress);

epilepsies with frequent attacks or the epileptic status in the anamnesis or mental disturbances;

diseases of endocrine system with moderate and considerable malfunction;

blood diseases in the presence of progressing signs;

active forms of tuberculosis of any localization, and also inactive tuberculosis in the next three years after disappearance of signs of activity;

chronic diseases of respiratory organs with respiratory (pulmonary) insufficiency of III degree;

bronchial asthma of heavy degree;

blood circulatory system diseases with heart failure of III or IV functional classes; stenocardias of tension of III or IV functional classes, aneurism of heart, returnable revmokardit;

hypertension with considerable dysfunction of "target organs";

the chronic hard proceeding progressing liver diseases;

malignant new growths irrespective of stage and results of treatment;

glaucomas, the progressing diseases of optic nerve, pigmentary degeneration of retina, lowering of visual acuity to 0,03 on both eyes without correction;

generalized periodontal disease and periodontosis of heavy degree.

II. Requirements to the state of health of the citizens enlisting under the contract and the citizens passing military service under the contract for determination of the validity to work, passing of military service in the territories which underwent to radioactive pollution owing to catastrophic crash on the Chernobyl NPP

4. The military-medical commissions take out the conclusion about the validity of the citizens enlisting under the contract and the citizens passing military service under the contract for work in the conditions of the increased risk of radiation damage (in exclusion zone, in settling out zone <*>) in the absence of the following mutilations at them, the diseases, conditions interfering passing of military service, work in the specified territories:

consequences of infectious and parasitic diseases of the central nervous system, organic damages of brain and spinal cord in case of the moderated and unsharply expressed residual phenomena with insignificant violation of functions;

consequences of injuries of brain and spinal cord, organic diseases of the central nervous system in case of moderate or insignificant violation of functions;

consequences of injuries and diseases of peripheral nerves in case of the permanent moderately expressed violations of functions;

diseases of endocrine system of medium-weight and easy form, obesity of II or III degrees, the lowered food;

system diseases of blood and the haematogenic bodies irrespective of nature of current and severity;

permanent changes of composition of peripheral blood (quantity of leukocytes of less 4,0 x/l or more 9,0 x/l, quantity of platelets of less 160,0 x/l, hemoglobin less than 120 g/l);

the persistent congenital and acquired immunodeficiency with clinical manifestations and confirmed with physical examination in stationary conditions in specialized departments of the medical organizations of the state and municipal health care systems or the military-medical organizations, infection of HIV;

sharp radiation sickness of any severity in the anamnesis and also received earlier in case of accident or accidental radiation of the effective dose exceeding 200 mzv within year;

chronic inflammatory diseases of kidneys and urinary tract in case of moderate malfunction, urolithic disease with frequent renal gripes and (or) with frequent aggravations (two and more times a year);

chronic diseases (consequences of damages) of joints, muscles, sinews, backbone, basin bones in case of moderate violation of functions or with frequent aggravations (two and more times a year);

chronic or habitual dislocations in the large joints arising in case of insignificant physical activities;

defects of fingers of hands or legs with malfunction substantially;

lack of the upper or lower extremity at any level;

malignant new growths irrespective of stage of disease and results of treatment, pretumoral diseases;

chronic recidivous inflammatory diseases of female genitals with moderate and considerable malfunction;

acute disease or exacerbation of chronic disease;

the expressed forms of respiratory allergoz, atopic dermatitis, eczema.

III. Requirements to the state of health of the citizens passing military service under the contract for determination of the validity to passing of military service in foreign states (except for foreign states with adverse hot climate)

The military-medical commissions take out the conclusion about the validity to passing of military service in foreign states (except for foreign states with adverse hot climate) the citizens who are passing military service under the contract, leaving in long (over 3 months) of business trip in the specified foreign states, in the absence of the following mutilations at them, the diseases interfering passing of military service in these foreign states:

acute disease or exacerbation of chronic disease;

the mutilations, diseases provided by articles 6-b, 6-v, 7-b (before treatment), 11-v, 32-v, 33-b, 39-v, 54-v (before treatment), 55-b, 60-b, 62-g, 74-v, 75-v, 76-v, 80-v, 83-b, 88-b schedules of diseases;

the diseases, conditions which are contraindication for carrying out preventive inoculations against diseases which are required for stay in the corresponding foreign state;

mental and behavioural disturbances (except lungs and moderately expressed, short-term neurotic, somatoformny frustration and frustration connected with stress);

epilepsies with frequent attacks or the epileptic status in the anamnesis or mental disturbances;

the heavy or progressing organic diseases of the central nervous system (vascular damages of brain with resistant focal symptomatology, multiple sclerosis, side amyotrophic sclerosis, cerebral palsy, siringomiyeliya, myopathy, myasthenia, consequences of the postponed tubercular meningitis), and also diseases and injuries of peripheral nervous system with moderate and considerable dysfunction of extremity;

hypertension with arterial hypertension of III degree;

stomach ulcer of stomach, duodenum with frequent aggravations (2 and more times a year);

chronic diseases of kidneys 3-5 stages;

ankiloz of two and larger joints, anchylosis of coxofemoral joint;

high amputation of hip;
