About approval of the Procedure for forming of the sheet about compensation payment to employers and sheet forms of compensation payment to employers

1. Approve the Procedure for forming of the sheet about compensation payment to employers of costs in the amount of single fee for obligatory national social insurance by fund of social protection of disabled people which is applied.
2. Approve sheet form of compensation payment to employers of costs in the amount of single fee for obligatory national social insurance by fund of social protection of disabled people which is applied.
3. To department of the labor market and employment (Komarova A. S.) submit this order on state registration to the Ministry of Justice of Ukraine.
4. This order becomes effective from the date of its official publication.
5. To impose control over the implementation of this order on the deputy minister - the chief of staff of Ivankevich V. V.

1. This Procedure determines the mechanism of forming and transfer by district, city, district and district jobcenters of public service of employment in the cities (further - jobcenters) sheets about compensation payment to employers of costs in the amount of single fee for obligatory national social insurance by fund of social protection of disabled people (further - the sheet) to territorial department of Social Security Fund of disabled people.
2. Jobcenters to the 10th following after month of submission of the report on the amounts of accrued payroll (the income, cash cover, the help, compensation) and the amounts of the assessed single contribution on obligatory national social insurance, taking into account data of the employer (further - the report) and the data of the Unified State Register of Legal Entities and physical persons - entrepreneurs, create insured persons and transfer the sheet to jobcenters in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol in the established form.
3. Jobcenters in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol to the 15th following after month of submission of the report transfer the generalized data to territorial department of Social Security Fund of disabled people for compensation payment to employers of costs in the amount of single fee to obligatory national social insurance.
