About approval of regulations on procedure for production of forms of licenses and about procedure for forming and maintaining the register of licenses for the territories of the Republic of Uzbekistan

In pursuance of the Law of the Republic of Uzbekistan "About licensing of separate types of activity" and the Presidential decree of the Republic of Uzbekistan of June 2, 2000 the No. UP-2612 "About Measures for Program Implementation for Liberalization and Deepening of Reforms in Political, Economic and Spiritual Spheres of Society, to Safety of the Country" Cabinet of Ministers decides:

1. Approve:

Regulations on procedure for production of forms of licenses according to appendix No. 1;

Regulations on procedure for forming and maintaining the register of licenses for the territories of the Republic of Uzbekistan according to the appendix N 2.

2. To impose control over the implementation of this resolution on the corresponding deputy prime ministers - heads of Complexes of the Cabinet of Ministers of the Republic of Uzbekistan.

Appendix № 1

to the resolution of the Cabinet of Ministers of December 20, 2000 No. 488

Regulations on procedure for production of forms of licenses

1. This Provision is developed in pursuance of the Law of the Republic of Uzbekistan "About licensing of separate types of activity", the Presidential decree of the Republic of Uzbekistan of June 2, 2000 No. UP-2612 "About Measures for Program Implementation for Liberalization and Deepening of Reforms in Political, Economic and Spiritual Spheres of Society, to Safety of the Country" also determines procedure for production of forms of licenses.

2. Licenses are drawn up on the corresponding forms. Forms of licenses are documents of the strict reporting, shall have accounting series, number and degree of security.

3. In each form of licenses shall be specified:

license number;

date of issue;

license effective period (for the licenses granted on limited effective period);

owner of the license;

location (postal address), identification taxpayer number and other details of the owner of the license;

the body which granted the license;

number according to the register of the license;

type of the licensed activities.

The license is signed by person authorized on that, sealed the organization which granted the license.

4. Production of forms of licenses is performed by the printing companies which were registered by the Uzbek agency on seal and information of the Republic of Uzbekistan, having permission of law-enforcement bodies and included in the list of the companies having the right to printing of forms of strict accounting.

5. The licensing body in case of development of the license shall approve in accordance with the established procedure the form of the license and attach to it the certificate which shall contain the following information:

total characteristic of design;

elements of graphical and chemical protection;

characteristic of paper (type, brand and density of paper).

6. Order placement on production of forms of licenses is made by the printing company based on the written request of the customer.

The sample of the form of the license approved in accordance with the established procedure and the copy of the certificate encloses to the letter (request).

7. All orders for production of forms of licenses are registered in the separate "Order book of forms of the licenses" numbered, which are strung together the under seal, signed head of the printing company.

8. After production of forms of licenses printing forms and the rejected forms are destroyed in the presence of the customer representative about what the statement is drawn up and the mark in "The order book of forms of licenses" becomes.

9. Production of forms of licenses is performed according to the established requirements for the mode of safety of documents of strict accounting.

10. Responsible for the mode of safety it is appointed the order of the head of the printing company.

11. After delivery of forms of licenses to the customer one copy of the form of the license and the approved sample remain on permanent storage in the printing company.

Regulations on procedure for forming and maintaining register of licenses for the territories of the Republic of Uzbekistan

1. This Provision is developed in pursuance of the Law of the Republic of Uzbekistan "About licensing of separate types of activity", the Presidential decree of the Republic of Uzbekistan of June 2, 2000 No. UP-2612 "About Measures for Program Implementation for Liberalization and Deepening of Reforms in Political, Economic and Spiritual Spheres of Society, to Safety of the Country" also determines procedure for maintaining by licensing authorities of the register of the licenses issued to legal entities and physical persons.

2. The register of licenses - set of databases of the licensing authorities containing data about the issued, suspended, renewed, renewed, cancelled licenses, and also about licenses which are terminated.

3. Licensing authorities when implementing the procedures connected with maintaining the register are guided by the Law "About Licensing of Separate Types of Activity", regulations on licensing of separate types of activity, other acts of the legislation of the Republic of Uzbekistan and this Provision.

4. Production of forms of licenses is performed by the printing companies which were registered by the Agency of information and mass communications, information directorates and mass communications of the Republic of Karakalpakstan, areas and the city of Tashkent, having permission of law-enforcement bodies and included in the list of the companies having the right to printing of forms of strict accounting.

5. Financing sources of the procedures connected with maintaining the register are the license fees established by the Cabinet of Ministers of the Republic of Uzbekistan on each type of activity and also payment for provision of information from the register of licenses in the form of statements about specific licensees.

6. Licensing authorities keep registers of licenses separately for each type of activity which licensing they perform. Data on licensing are entered in the register of licenses in day of decision making about licensing to business entity. All marks date and certified of the register of licenses by sign and seal.
