In pursuance of the Law of the Republic of Uzbekistan "About licensing of separate types of activity", and also according to the Regulations on procedure for production of forms of licenses approved by the resolution of the Cabinet of Ministers of the Republic of Uzbekistan of December 20, 2000 N 488, the Cabinet of Ministers decides:

1. Approve:

the form of the license for implementation of the separate types of activity licensed by the Cabinet of Ministers of the Republic of Uzbekistan according to appendix No. 1;

The instruction about procedure for storage, accounting, issue and write-off of forms of licenses, and also their registrations in the office of the Cabinet of Ministers of the Republic of Uzbekistan according to appendix No. 2.

2. To the managing director of the office of the Cabinet of Ministers of the Republic of Uzbekistan:

provide the organization of work on storage, accounting, issue and write-off of forms of licenses, and also their registration in the office of the Cabinet of Ministers of the Republic of Uzbekistan according to the approved Instruction;

approve distribution of series of forms of licenses on types of activity on which licenses are drawn up in the office of the Cabinet of Ministers of the Republic of Uzbekistan.

3. To management of economic activity of the office of the Cabinet of Ministers of the Republic of Uzbekistan:

together with the First department of the Cabinet of Ministers of the Republic of Uzbekistan to provide placement of requests for production of necessary number of forms of the license in printing factory of GPO "Davlat of Belgisa";

by the calculations provided by printing factory to GPO "Davlat of Belgisa" to make payment of the produced forms of licenses;

on the basis of requests of the office of the Cabinet of Ministers of the Republic of Uzbekistan to provide production of registers of licenses in necessary quantity.

4. To the first department and Management of legal providing the Cabinet of Ministers of the Republic of Uzbekistan in accordance with the established procedure to provide the made forms of licenses from printing factory of GPO "Davlat of Belgisa", their storage, conducting strict accounting, issue and write-off.

5. To heads of the structural divisions of Executive office of the Cabinet of Ministers performing execution of licenses in week time to introduce in the First department of the Cabinet of Ministers of the Republic of Uzbekistan offers on production of registers of licenses on each licensed type of activity.
