1. Approve the enclosed form of the state statistical reporting 4-tek (fuel) "Report on remaining balance, receipt and fuel consumption" of quarter frequency and to enact it starting with the report for January-March, 2015.
2. Approve the enclosed Instructions on filling of form of the state statistical reporting 4-tek (fuel) "Report on remaining balance, receipt and fuel consumption" and to enact them starting with the report for January-March, 2015.
3. Extend the form of the state statistical reporting specified in Item 1 of this resolution on legal entities (except the Instructions listed in part four of Item 1 on filling of form of the state statistical reporting 4-tek (fuel) "Report on remaining balance, receipt and fuel consumption", approved by this resolution), their separate divisions having separate balance, consuming or realizing fuel and the oil products including gas liquefied and natural.
4. Recognize invalid:
