Procedure for maintaining the unified register of the lottery terminals and the scope of information included in the unified register of lottery terminals

1. This Procedure establishes rules of maintaining the unified register of the lottery terminals by the Federal Tax Service and the scope of information included in the unified register of lottery terminals (further - the Register).

2. The Register joins data on the lottery terminals used when carrying out drawing lottery (further - lottery).

3. For inclusion of data on lottery terminals in the Register the operator of lottery who is going to use lottery terminals directs to the Federal Tax Service the application for inclusion of data in the unified register of lottery terminals (further - the statement) containing the following data:

1) complete and (if is available) the abbreviated name, including trade name, form of business of the operator of lottery, the place of its stay, identification taxpayer number, the state registration number of record about creation of the operator of lottery;

2) the name of lottery when which carrying out use of lottery terminals is planned;

3) date and number of the decision on carrying out lottery;

4) terms of carrying out lottery;

5) the name of lottery terminals, with indication of the manufacturer and country of source;

6) number of lottery terminals;

7) addresses of the locations of lottery terminals with indication of their serial numbers.

The application is signed by the head of the operator of lottery or person authorized by it and certified by seal the operator of lottery (in the presence of seal).

4. The following documents are enclosed to the application:

a) the copy of the contract for carrying out lottery with the organizer of the All-Russian state lottery signed by the operator of lottery (further - the organizer of lottery);

b) the copy of the document containing the description and technical characteristics of the lottery terminal;

c) the copy of the document confirming the right to use the lottery terminal;

d) the copy of technical data sheet of the manufacturer of the lottery terminal, including the document of the manufacturer of the lottery terminal containing the description of operation of such lottery terminal;

e) the copy of the certificate confirming results of examination of the lottery terminal for the purpose of establishment of absence in it the hidden (not declared) opportunities for change of technical characteristics of the lottery terminal.

The copies of documents specified in this Item of the Procedure are certified by the operator of lottery.

5. The statement and documents attached to it go the operator of lottery to the Federal Tax Service within 15 days from the date of the conclusion the operator of lottery of the contract for carrying out lottery with the organizer of lottery.

6. Inclusion of data on lottery terminals in the Register is performed within 30 days from the date of receipt of the Procedure for the statement specified in Item 3 presents in the Federal Tax Service about what the corresponding entry in the Register is made.

7. The Register contains the following data:

1) sequence number of accounting record;

2) date of introduction of accounting record;

3) the name of lottery terminals with indication of the name of the manufacturer and country of source;

4) addresses of the locations of lottery terminals with indication of their serial numbers;

5) the name of lottery when which carrying out lottery terminals are used;

6) date and number of the decision on carrying out lottery;

7) terms of carrying out lottery;

8) complete and (if is available) the abbreviated name, including trade name, form of business of the operator of lottery, the place of its stay, identification taxpayer number, the state registration number of record about creation of the operator of lottery;

9) date of change of the locations of lottery terminals;

10) date and basis of the termination of use of lottery terminals.

8. The bases for inclusion of changes in the Register are:

change of the locations of lottery terminals;

use of lottery by the operator when carrying out lottery of new lottery terminals;

termination of use of lottery terminals.

9. In case of change of the locations of lottery terminals the operator of lottery directs the application containing the following data to the Federal Tax Service:

1) complete and (if is available) the abbreviated name, including trade name, form of business of the operator of lottery, the place of its stay, identification taxpayer number, the state registration number of record about creation of the operator of lottery;

2) the name of lottery when which carrying out lottery terminals are used;

3) date and number of the decision on carrying out lottery;

4) terms of carrying out lottery;

5) the name of lottery terminals with indication of the name of the manufacturer and country of source;

6) new addresses of the locations of lottery terminals with indication of their serial numbers;

7) date of change of the locations of lottery terminals.

The application is directed by the operator of lottery to the Federal Tax Service within 10 days from the moment of adoption of the relevant decision.

10. If the operator of lottery when carrying out lottery plans use of new lottery terminals, the operator of lottery directs to the Federal Tax Service the application specified in Item 3 presents of the Procedure and documents which list is stipulated in Item 4 presents of the Procedure.
