About approval of form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children" and the instruction for its filling 

According to article 3 of the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", the subitem 5 of item 4 of the Regulations on the Ministry of social policy of Ukraine approved by the Presidential decree of Ukraine of April 6, 2011 No. 389 (with changes), PRIKAZYVAYU:

1. Approve form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children" and the Instruction for its filling which are applied.

2. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of September 1, 2003 No. 241 "About approval of form of the reporting on provision of the government public assistance to disabled people since the childhood and to handicapped children and the instruction for its filling", registered in the Ministry of Justice of Ukraine on September 18, 2003 for No. 823/8144 (with changes).

3. To provide to department of the government public assistance (Muzychenko V. V.):

1) submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure;

2) representation of statistics of Ukraine to Public service of summary information in general on Ukraine and administrative and territorial units annually till March 1 for the accounting period.

4. To provide to the minister of social policy of the Autonomous Republic of Crimea, heads of structural divisions of social protection of the population of the regional, Kiev and Sevastopol city public administrations:

1) collection, processing and generalization of information on the GSPI form;

2) submission of the summarized information in the GSPI form at the scheduled time in the Ministry of social policy of Ukraine and territorial authorities of the state statistics.

5. To impose control over the implementation of this order on the First Deputy Minister of social policy of Ukraine Mushchinin V. V.

6. This order becomes effective from the date of its official publication.

Instruction for filling of form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children"

1. For ensuring operating control behind condition of implementation of the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children" structural divisions concerning social protection of the population Kiev and Sevastopol of public administrations, executive bodies, district, district in, city, district in the cities (except of Kiev and Sevastopol) councils, the centers for charge and payment of the help constitute form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children" (further - the report on the GSPI form) for the accounting period once a month and the 5th after the accounting period give to the Ministry of social policy of the Autonomous Republic of Crimea, structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations.

2. The ministry of social policy of the Autonomous Republic of Crimea, structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations to the 15th after the accounting period submit the report on the GSPI form to the Ministry of social policy of Ukraine and territorial authorities of the state statistics.

The ministry of social policy of Ukraine in March 1 provides to Public service of statistics of Ukraine the consolidated information on Ukraine and administrative and territorial units concerning provision of the government public assistance to disabled people since the childhood and to handicapped children.

3. When filling the report on the GSPI form those are used to unit of measure which are specified in shape.

4. Provided in the report on the GSPI form of information the head of body, constituted according to the current legislation is responsible for reliability.

5. Accounting of number of receivers of the government public assistance to disabled people since the childhood and to handicapped children and accounting of the amounts of this help are performed by structural divisions concerning social protection of the population based on personal records of receivers of the government public assistance or based on request to databases of receivers of this type of the help.

6. In columns with 1 on 4 information of rather total quantity of receivers of the government public assistance to disabled people since the childhood and to handicapped children in general and separately on certain periods is specified.

In columns with 5 to 14 information on the total amount of payment of the government public assistance to disabled people since the childhood and to handicapped children in general and separately on certain periods is specified.

7. In column 1 the total quantity of receivers of the government public assistance to disabled people since the childhood and to handicapped children, including persons who are on complete state content, as of the end of the accounting period taking into account those disabled people to whom the help in reporting is appointed month (updated) is specified (this number does not join disabled people to whom payment was stopped, the quantity corresponds to the number of receivers on acting with personal accounts).
