About approval of sample of the form, technical description and the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid, destructions of the certificate of the stateless person for trip abroad with the contactless electronic medium
According to part two of article 15 of the Law of Ukraine "About the Unified state demographic register and documents confirming citizenship of Ukraine, proving the identity or her special status" the Cabinet of Ministers of Ukraine decides:
1. Approve:
sample and the technical description on the form of the certificate of the stateless person for trip abroad with the contactless electronic medium according to appendices 1 and 2;
Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid, destructions of the certificate of the stateless person for trip abroad with the contactless electronic medium which is applied.
2. Determine that:
the documents issued before entry into force of this resolution are effective during the term for which they were issued;
before introduction, registrations and certifications of the stateless person for trip abroad with the contactless electronic medium which sample of the form is approved by this resolution the certificate are drawn up with use of forms of the previous samples.
3. Enter registration and certification of the stateless person for trip abroad with the contactless electronic medium which sample of the form is approved by this resolution, using means of the Unified state demographic register.

The technical description on the form of the certificate of the stateless person for trip abroad with the contactless electronic medium
1. The form of the certificate of the stateless person for trip abroad with the contactless electronic medium (further - the form of the certificate) is made in the form of book of 88 x 125 in size (± 0,75) of millimeters.
2. The form of the certificate consists of soft cover, the fly-leaf, the page of data and 16 paper pages.
The contactless electronic medium is implanted into the right part of cover. The contactless electronic medium meets the requirements of the ISO/IEC 14443 standard for record and data read-out and requirements of ICAO, established to electronic documents.
