According to the subitem 3) 6, the subitem 11) of article 9 of the Law of the Republic of Kazakhstan of April 15, 2013 "About the state services" the Government of the Republic of Kazakhstan DECIDES: Articles

1. Approve enclosed:

1) standard of the state service "State Registration and Re-registration of Products of Baby Food, Food and Dietary Supplements to Food, Genetically Modified Objects, Dyes, Means of Disinfection, Disinsection and Deratization, Materials and Products Contacting to Water and Food, Chemicals, the Separate Types of Products and Substances Making Harmful Effects on Health of the Person";

2) standard of the state service "Issue of the Sanitary and Epidemiologic Conclusions about Compliance (Discrepancy) of Object of the State Sanitary and Epidemiological Surveillance to Regulatory Legal Acts in the Sphere of Sanitary and Epidemiologic Wellbeing of the Population and to Hygienic Standard Rates";

3) standard of the state service "Assignment of Accounting Numbers to Objects of Production of the Food Products Which Are Subject to Sanitary and Epidemiological Surveillance";

4) standard of the state service "Issue of the Sanitary and Epidemiologic Conclusion on Projects, Products, Works and Services";

5) standard of the state service "Issue of Permission to Microbiological Laboratories for Work with Activators 1-4 Groups of Pathogenicity";

6) standard of the state service "Coordination of Expiration Dates and Storage Conditions of Food Products".

2. Recognize invalid some decisions of the Government of the Republic of Kazakhstan according to appendix to this resolution.

3. This resolution becomes effective after ten calendar days after day of its first official publication.

1. General provisions

1. The state service "State Registration and Re-registration of Products of Baby Food, Food and Dietary Supplements to Food, Genetically Modified Objects, Dyes, Means of Disinfection, Disinsection and Deratization, Materials and Products Contacting to Water and Food, Chemicals, the Separate Types of Products and Substances Making Harmful Effects on Health of the Person" (further - the state service).

2. The standard of the state service is developed by consumer protection Agency of the Republic of Kazakhstan (further - the Agency).

3. The state service appears the Agency and its territorial subdivisions (further - the service provider).

Documents acceptance and issue of results of rendering the state service are performed:

1) through office of the service provider;

2) through the web portal of "the electronic government": www.e.gov.kz.

4. Term of rendering the state service by the service provider:

1) from the moment of delivery of document package uslugopoluchatel, and also in case of the address on the portal:

in case of issue of the certificate on state registration - no later than 30 (thirty) calendar days;

in case of re-registration of the certificate on state registration - 30 (thirty) calendar days;

2) the most allowed time of expectation for delivery of document package - 15 (fifteen) minutes;

3) the most allowed time of servicing of uslugopoluchatel - 15 (fifteen) minutes.

5. Form of rendering the state service: electronic and (or) paper.

Result of rendering the state service - the certificate on state registration (re-registration) of products of baby food, food and dietary supplements to food, genetically modified objects, dyes, means of disinfection, disinsection and deratization, materials and products contacting to water and food, chemicals, the separate types of products and substances making harmful effects on the health of the person which is the form of the strict reporting.

In case of the address of uslugopoluchatel to the service provider - the result of rendering the state service is drawn up on paper, unpacked and certified by seal and the signature of the head of the service provider.

In case of the address of uslugopoluchatel through the portal - the notification on result of the state service goes to "private office" in electronic form, certified by the digital signature (further - the EDS) the authorized person of the service provider. to the service provider:

the statement of the established form according to appendix 1 to this standard of the state service;

form of data on the document for receipt of the certificate on state registration according to appendix 2 to this standard of the state service;

copies of documents according to which products are made (standards, specifications, regulations, technological instructions, specifications, compoundings, the information about structure), certified by the manufacturer (producer);

the copy of the written notice of the manufacturer (producer) that products (product samples) made by it meet the requirements of documents according to which it is made;

copies of documents of the manufacturer (producer) on use (operation, use) of controlled goods (the instruction, management, regulations, recommendations) or its copy certified by uslugopoluchatel);

copies of labels (packaging) or their prototypes on the controlled goods certified by uslugopoluchatel;

the copies of documents on specific activity of dietary supplement to food (for the medicines containing unknown components, unofficial copy-books) certified by uslugopoluchatel;

copies of protocols of researches (testing), (acts of hygienic examination), scientific reports, expert opinions.

In addition for the controlled goods produced out of customs area of state members of the Customs union (further-) it are provided to TS:

the copies of documents on the toxicological characteristic of medicine (for pesticides, agrochemicals, remedies and regulators of growth of plants) certified according to the legislation of state members of TS in which state registration is carried out;

the copy of the document, competent authorities of health care (other state authorized bodies) of the country in which dietary supplement to food is made, nutritional supplement, disinfecting (disinsection, deratization) means, cosmetic products confirmatory safety and resolving free circulation of these products in the territory of the state of the manufacturer (producer), certified according to the legislation of state members of TS in which registration, or data on lack of need of execution of such document is carried out;

copies of the documents confirming import of samples of controlled goods to customs area of state members of TS, the TS certified according to the legislation of state members in which state registration is carried out;

on the portal:

1) request in electronic form, the uslugopoluchatel certified by the EDS;
