According to the subitem 1) Item 2 of article 25 of the Law of the Republic of Kazakhstan of May 21, 2013 "About personal data and their protection" PRIKAZYVAYU:

1. Approve the enclosed List of personal data necessary and sufficient for accomplishment of the performed tasks.

2. To legal support department (Kuang A. K.) provide in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) after state registration of this order its official publication in mass media and placement on official Internet resource of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan.

3. To impose control of execution of this order on the responsible secretary of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan Kurmangaliyeva A. D.

4. This order becomes effective since November 25, 2013 and is subject to official publication.

Data on the income: about the average monthly income from January 1, 1995 for any 3 years in a row, the archive help with indication of data on earnings or the electronic copy of the archive document certified by the electronic digital signature of the authorized employee of the Public Records Office or departmental archive, the help about the income amounts paid to physical person and performed from the income of compulsory pension contributions or the document on the income of individual entrepreneurs, lawyers, private notaries, private legal executives and professional mediators issued by body of state revenues.

The data confirming working life: the data containing in the service record; to the help of archive organizations, the electronic copy of archive documents, the witnessed electronic digital signature of the authorized employee of the Public Records Office or departmental archive, or from place of employment in case of absence or worthlessness of the service record, lack of the corresponding employment records, availability of corrections, inaccuracies, discrepancies; the marriage certficate (the help about civil registration) either the certificate on annulment of marriage or the statement from assembly record about marriage (in case of change of surname).
