1. Approve enclosed:

1.1. Standard form of the plan of sanitation of the debtor on the case of bankruptcy.

1.2. Requirements concerning plan development of sanitation of the debtor according to the Standard form of the plan of sanitation of the debtor on the case of bankruptcy.

1.3. Standard form of the voluntary settlement on the case of bankruptcy.

1.4. Requirements concerning development of the voluntary settlement of the debtor according to the Standard form of the voluntary settlement on the case of bankruptcy.

2. To department of notariate, bankruptcy and functioning of the central zaveritelny body (Chizhmar K. I.) submit this order on state registration according to the Presidential decree of Ukraine of October 03, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".

3. This order becomes effective from the date of its official publication.

4. To impose control over the implementation of this order on the director of the department of notariate, bankruptcy and functioning of the central zaveritelny body Chizhmar K. I.

Requirements 
on plan development of sanitation of the debtor according to the Standard form of the plan of sanitation of the debtor on the case of bankruptcy

1. These Requirements parts one of article 3 of the Law of Ukraine "About recovery of solvency of the debtor or recognition by his bankrupt" for the purpose of the unified approach to plan development of sanitation of the debtor according to the Standard form of the plan of sanitation of the debtor on the case of bankruptcy are developed according to the paragraph the eighth (further - the Plan of sanitation). These Requirements contain the description of technique of filling of Sections of the Plan of sanitation.

2. The plan of sanitation is drawn up in the form of the systematized list of interconnected and consecutive actions which are subject to implementation during proceeedings about bankruptcy for the purpose of prevention of recognition of the debtor by the bankrupt or his liquidation which are directed to improvement of financial and economic provision of the debtor, and also to satisfaction of requirements of creditors, with indication of terms of implementation of such actions, with anticipation of their expected results (consequences).

3. Execution of the Plan of sanitation is performed in document type which contains details, obligatory for it, namely: date and document number, number of proceeedings about bankruptcy, full name of hozyayenny court, the name of the debtor, contents of the Plan of sanitation, the title to the text, the signature (signatures), signature stamps of approvals (coordination, approvals), the list of appendices. In case of registration of stvplan of sanitation use of other details is possible if it is required by its content.

4. The written statement of the Plan of sanitation of the debtor is performed by means of the printing means. For printing of the Plan of sanitation the font of Times New Roman, font of 12-14 printing Items in size (except titles) and paper of the A4 format (210 x 297 mm) is used.

5. Pages of the Plan of sanitation are subject to obligatory numbering. Numbering of pages of the Plan of sanitation begins with the second.

6. The drawn-up Plan of sanitation shall be stitched with podkleivaniye on its turnover of paper mark: "It is numbered, strung together and fastened (the number (the Arab figures and words) of sheets)". The specified mark is signed by the managing director of sanitation (the manager of property) and sealed the arbitration manager with placement of parts of the sign and seal on the back of the Plan of sanitation and their other parts - on the glued paper mark.

7. Approval (approval, coordination) of the Plan of sanitation is drawn up by the corresponding signature stamp on the face of the title page of the Plan of sanitation. The plan of sanitation contains only those from approval signature stamps (approval, coordination) which are stipulated by the legislation for specific case on bankruptcy.

8. In the paragraph the second the Section I of the Plan of sanitation "The introduction (preface)" should be specified the generalized statement of the purpose of sanitation, essence of the actions for recovery of solvency of the debtor, methods, means and the expected results (consequences) of accomplishment of the Plan of sanitation provided by the Plan of sanitation.

9. General information concerning case on bankruptcy is given in the Section ІІ (date opening of proceeedings about bankruptcy, file number about bankruptcy, the name of court, the creditor who initiated opening of proceeedings about bankruptcy, information on the register of requirements of creditors, the general size of requirements of creditors and so forth).
