According to part two of article 22 of the Law of Ukraine "About state registration of legal entities and physical persons - entrepreneurs", Item 3 of the Regulations on the Ministry of Justice of Ukraine approved by the Presidential decree of Ukraine of April 06, 2011 No. 395, I ORDER:

1. Approve the applied forms and contents of messages concerning entering of data into the Unified State Register of Legal Entities and physical persons - the entrepreneurs who are subject to obligatory publication in specialized printing mass media:

1.1. Message on carrying out state registration of the legal entity.

1.2. Message on change of the location of the legal entity.

1.3. Message on change of the name of the legal entity.

1.4. Message on acceptance by founders (participants), court or authorized body of the decision on allocation.

1.5. The message on acceptance by founders (participants) or the body of the decision authorized by them concerning the termination of the legal entity.

1.6. Message on loss of originals of constituent documents.

1.7. Message on staff of the commission on the termination of the legal entity (commission on reorganization, liquidation commission) and its chairman or appointment of the liquidator.

1.8. The message on the resolution of the judgment concerning the termination of the legal entity if such decision is not connected with bankruptcy of the legal entity.

1.9. The message on the resolution of the judgment concerning initiation of proceeedings on bankruptcy of the legal entity.

1.10. Message on reduction of the authorized (share) capital of the legal entity.

1.17. The message on carrying out state registration of the termination of business activity of the physical person - the entrepreneur.

1.18. The message on carrying out state registration of the termination of business activity of physical person - the entrepreneur by the principle of acquiescence.

1.19. The message on the resolution of the judgment concerning suit abatement on bankruptcy of the legal entity.

1.20. The message on the resolution of the judgment concerning suit abatement on bankruptcy of the physical person - the entrepreneur.

2. To provide to the public registration service of Ukraine (D.M.'s Crow) reduction by the state company "Information and Resource Center" (Quiricus Yu. G.) software of maintaining the Unified State Register of Legal Entities and physical persons - entrepreneurs in compliance with forms and contents of the messages approved by this order within three months from coming into force of this order.

3. To department of interaction with authorities (Martynenko S. V.) to submit this order on state registration according to the Presidential decree of Ukraine of October 03, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".

4. This order becomes effective from the date of its official publication, except Item 1 of this order which is becoming effective in 3 months from the date of its official publication.

5. To impose control over the implementation of this order on the Chairman of the Public registration service of Ukraine D.M.'s Crow and the acting director of the department of interaction with authorities Martynenko S. V.
