1. Bring in Item 5 of the list of documents which are enclosed to the application for licensing for the separate type of economic activity approved by the resolution of the Cabinet of Ministers of Ukraine of July 4, 2001 No. 756 (The Official Bulletin of Ukraine, 2001, No. 27, by Art. 1212; 2011, No. 52, Art. 2070; 2012, No. 58, Art. 2336), such changes:

in the column "Type of Economic Activity" to add Item with words ", import of medicines";

in the column "Documents Which Are Enclosed to the Application for Licensing for Separate Type of Economic Activity" to add Item with paragraphs of such content:

"import of medicines

the copy of the file of the importer (in the form established by licensed conditions) approved as the subject of managing".

2. To the Ministry of Health to bring in two-month time own regulatory legal acts into accord with this resolution.

3. This resolution becomes effective since December 1, 2013, except Item 2, which becomes effective from the date of its publication.
