3. To department of legal service of the Ministry of Health of the Republic of Kazakhstan (Danayeva Zh. Zh.) provide official publication of this order in mass media after its state registration in the Ministry of Justice of the Republic of Kazakhstan.

4. To impose control of execution of this order on the Vice-Minister of Health of the Republic of Kazakhstan Bayzhunusov E. A.

Regulations of rendering the state service "Issue of Documents on Passing of Preparation, Advanced Training and Retraining of Personnel of Industry of Health Care"

1. General provisions

1. These regulations of rendering the state service "Issue of Documents on Passing of Preparation, Advanced Training and Retraining of Personnel of Industry of Health Care" (further - Regulations) are developed according to item 4 of article 9-1 of the Law of the Republic of Kazakhstan "About ministerial procedures".

2. The state service "Issue of Documents on Passing of Preparation, Advanced Training and Retraining of Personnel of Industry of Health Care" (further - the state service) is rendered by the scientific organizations and the organizations of education in the field of health care realizing programs of technical and professional, postsecondary, higher, postgraduate and additional professional education (further - the organizations).

3. Form of the rendered state service: not automated.

4. The state service is rendered based on item 4 of article 175 of the Code of the Republic of Kazakhstan of September 18, 2009 "About health of the people and health care system", the Standard of the rendered state service "Issue of Documents on Passing of Preparation, Advanced Training and Retraining of Personnel of Industry of Health Care", No. 1575 approved by the order of the Government of the Republic of Kazakhstan of December 11, 2012 (further - the Standard).

5. Issue is result of the rendered state service:

1) according to professional training programs of technical and professional, postsecondary education - the diploma;

2) according to professional training programs of the higher education - the diploma;

3) upon termination of internship - the certificate;

4) according to professional training programs of postgraduate education, upon termination of magistracy and doctoral studies - the diploma, upon termination of residency - the certificate;

5) according to programs of additional professional education upon termination of retraining - the certificate, upon termination of advanced training - the certificate.

6. The state service is rendered to the physical persons which finished training in the organization and passed final assessment (further - the receiver of the state service).

7. In the course of rendering the state service participation of other state bodies is not provided.

2. Requirements to rendering the state service

8. Information on the location and the working schedule of the organizations, procedure and the course of rendering the state service can be received on the Internet - resource of www.mz.gov.kz authorized body and the organizations.

9. For receipt of the document on passing of preparation, advanced training and retraining of personnel physical persons shall show documents, stipulated in Item 11 Standards.

10. Terms of rendering the state service are specified in Item 7 of the Standard.

11. The bases for suspension of provision of the state service are stipulated in Item 15 Standards.

3. The description of operations procedure (interaction) in the course of rendering the state service

12. Documents acceptance and receipt of information by the consumer on the state service is performed through structural divisions of the scientific organizations and the organizations of education in the field of health care.

13. Stages of rendering the state service:

1) the accepted documents are registered by the employee of the organization of education;

2) the registered documents go to the responsible person of structural division;

3) the employee contractor considers the submitted documents, processes the document on passing of preparation, advanced training and retraining of personnel, according to the order of the organization of education on issue of the document and reports to coordination to the supervising deputy manager of the organization of education.

4) after the resolution of the supervising deputy manager the document is transferred to the signature to the head;

5) after signing by the head, the document is transferred to registration for issue to the receiver of the state service.

6) the receiver of the state service receives the document purposely and undersigns for register.
