1. Approve enclosed:

single form of the certificate of conformity to requirements of the technical regulation of the Eurasian Economic Union and rule of its registration;

single form of the declaration of conformity to requirements of technical reglamentayevraziysky economic union and rule of its registration.

2. Declare invalid the Decision of the Commission of the Customs union of December 9, 2011 N 896 "About Single document forms about assessment (confirmation) of compliance (the declaration of conformity to technical regulations of the Customs union, the certificate of conformity to technical regulations of the Customs union)".

3. Determine that till July 1, 2014 application of the Single document forms about assessment (confirmation) of compliance approved by the Decision of the Commission of the Customs union of December 9, 2011 N 896 is allowed.

4. This Decision becomes effective after 30 calendar days from the date of its official publication.

II. Rules of execution of the certificate of conformity to requirements of the technical regulation of the Eurasian Economic Union

1. The certificate of conformity to requirements of the technical regulation of the Eurasian Economic Union (further respectively – the certificate, the Union) is drawn up by the certification body included in the unified register of conformity assessment bodies of the Union.

2. Ceased to be valid according to the Decision of Board of the Eurasian economic commission of 20.12.2022 No. 198

3. Forms of the certificate and appendix to it are documents of the strict reporting, are made in state members of the Union (further - state members) by typographical method and have at least 4 degrees of protection,
including:

a) gilyoshny frame of positive display;

b) the microtext placed on perimeter of gilyoshny frame;

c) translucent holographic protective element;

d) the typographical number consisting of series of the form, 2-unit alphabetic code of state member (according to the qualifier of the countries of the world) and sequence number of the form (7 Arab figures).

4. The certificate is filled in in Russian with use of electronic printers and in case of availability of the relevant requirement in the legislation of state member – in state language of state member in which certification of products is performed.

In case of filling of the certificate in Russian and state language of one of state members it is filled according to Item 6 of this Section on the different parties of the certificate.

If necessary the name of the manufacturer, its location (the address of the legal entity), and also the address (addresses) of the place of implementation of activities for production of products (except state) and designation of products (type, brand, model, the article, etc.) can be specified with use of letters of the Latin alphabet.

5. All fields of the certificate shall be filled (in the original certificate numbering of fields is absent).

6. In the certificate are specified:

a) in the field 1 – the texts executed in 2 lines in the following sequence:

1st line - "EURASIAN ECONOMIC UNION";

2nd line - "CERTIFICATE OF CONFORMITY";

b) in the field 2 – single market circulation mark of the Eurasian Economic Union;

c) in the field 3 – registration certificate number which is created according to the legislation of state member with indication of abbreviation of EEU (Eurasian Economic Union) and 2-unit alphabetic code of state member according to the qualifier of the countries of the world;

d) in the field 4 – the typographical number of the form of the certificate provided by the subitem "g" of Item 3 of this Section and which is put down in case of production of the form;

e) in the field 5 – full name of the certification body which issued the certificate, its location (the address of the legal entity), and also the address of the place of implementation of activities (if addresses differ), registration number and registration date of the certificate of accreditation of certification body, and also phone number and the e-mail address;

e) in the field 6 – full name of the applicant, its location (the address of the legal entity) and the address (addresses) of the place of implementation of activities (if addresses differ) – for the legal entity or surname, name and middle name (in the presence), the residence and the address (addresses) of the place of implementation of activities (if addresses differ) – for the physical person registered as the individual entrepreneur, and also registration or accounting (individual, identification) number of the applicant assigned in case of state registration of the legal entity or physical person registered as the individual entrepreneur according to the legislation of state members, phone number and the e-mail address;

g) in the field 7 – full name of the manufacturer, its location (the address of the legal entity) and the address (addresses) of the place of implementation of activities for production of products (if addresses differ) – for the legal entity and his branches which make products or surname, name and middle name (in the presence), the residence and the address (addresses) of the place of implementation of activities for production of products (if addresses differ) – for the physical person registered as the individual entrepreneur;

h) in the field 8 – data on products, including:

the name and designation (in the cases provided by technical regulations of the Union (technical regulations of the Customs union) (further – technical regulations)) products and (or) other symbol appropriated by the manufacturer of products (in the presence);

the name of products (in the cases provided by technical regulations) (in the presence);

other data on products providing its identification (in the presence);

the name and designation of the document (documents) according to which products (the standard, the standard of the organization, specifications or other document) are made (in the presence);
