About approval of the Instruction about activities of registry of the organization giving out-patient and polyclinic help

For the purpose of implementation of the State program of development of health care of the Republic of Kazakhstan "Salamatta Kazakstan" for 2011-2015, the Republic of Kazakhstan approved by the Presidential decree of November 29, 2010 No. 1113 and enhancement of delivery of health care to the population, ensuring availability of medical care, PRIKAZYVAYU:

1. Approve the enclosed Instruction about activities of registry of the organization giving out-patient and polyclinic help (further - the Instruction).

2. To provide to chiefs of Managements of health care of areas and cities of Astana, Almaty (in coordination) activities of registries of the organizations giving out-patient and polyclinic help according to the approved Instruction.

3. To impose control of execution of this order on the Vice-Minister of Health of the Republic of Kazakhstan Bayzhunusov E. A.

4. This order becomes effective from the date of its signing.

The instruction about activities of registry of the organization giving out-patient and polyclinic help

1. General provisions

1. This Instruction regulates the organization of activities of registry of the organization giving out-patient and polyclinic help (further - OAPP).

2. The registry is located on 1 floor of the building of OAPP. The workplace of registry is based on the principle "closer to the visitor" without glass partitions. Placement of registry shall provide the maximum availability to patients in receipt of services of registry from all directions (circular, fan arrangement of registry in new and buildings of OAPP under construction).

3. Entrance doors in OAPP shall provide free admission and exit for patients, and also OAPP shall have ramps for transportation of patients in wheelchair.

4. The registry of OAPP incorporates service desk.

5. The working hours of registry are performed from 8-00 till 20-00 o'clock without interruption.

2. Organization of work of registry of the organization of primary health care

§ 1. Registry

6. In registry of the organization of primary health care (further - PHC) services are rendered by medical registrars. The number of medical registrars is established according to the order of the Minister of Health of the Republic of Kazakhstan of April 7, 2010 No. 238 "About approval of standard states and regular standard rates of the organizations of health care" (1 position on 5 doctors conducting outpatient appointment).

7. The registry is equipped with the multichannel telephone communication providing continuous communication in working hours of the PHC organization.

8. As a part of registry the dispatching service equipped with phones for preliminary making an appointment to doctors, at least one, phones for call acceptance, at least one, phones for acceptance from service of emergency medical service of unreasonable challenges in working hours of the PHC organization, at least one is created.

9. Medical registrars render the services including:

1) registration and execution of necessary medical documentation of the PHC organization;

2) the organization of acceptance by specialists strictly according to the procedure according to record time from 8-00 till 20-00 o'clock on a rolling basis, except as specified, requiring rendering ambulance and emergency medical service;

3) preliminary making an appointment with the doctor in the conditions of the PHC organization and challenge on the house by record, self-record, in case of the direct address, by phone and through websites of the PHC organization;

4) maintaining the magazine of preliminary record with indication of date and time of acceptance of the specialist;

5) acceptance from service of emergency medical service of unreasonable challenges in working hours of the PHC organization;

6) transfer of unreasonable challenges of emergency medical service on sites;

7) forming of the register of the attached population, including in electronic format, in the absence of attachment the procedure of attachment in accordance with the established procedure is carried out;

8) ensuring matching and delivery of out-patient cards in pre-medical, viewing and medical offices;

9) correct maintaining and storage of card-index.

10. The challenge of the doctor on the house the medical registrar is registered in "The book of record of challenges of the doctor on the house" (form 031/at), search of out-patient cards for the doctor is performed. Call acceptance comes to an end in 2 hours prior to completion of work of the PHC organization (till 18:00 o'clock). In cases when the medical registrar finds it difficult to make the independent decision on justification of challenge of the doctor on the house, it is necessary to inform of it the manager of department of the Center of family health (local service).

11. Servicing of challenges at home by the doctor or the average health worker is coordinated by the manager of department of local (all-medical) service, in case of its absence the district doctor (general practitioner).

12. Medical registrars bear responsibility for safety of documentation.

13. The card-index is in the place unavailable to strangers.

14. In case of complete computerization maintaining electronic and cancellation of paper document flow, with the subsequent transfer of out-patient maps to patients is allowed.

15. For work with elderly people, disabled people, is determined responsible of number of persons employed of registry (or separate registry in the presence of the areas) which specifies the reason of the address and takes measures for their further servicing.

16. In registry issue of references, results of researches, sheets of disability shall not be performed.

17. Control of work of registry of the PHC organization is exercised by service of internal audit.

18. The wall evident information placed in a visible place in the hall near registry includes:

1) rules of attachment to the PHC organization with indication of standard rates of number of the attached population on one doctor of PHC;

2) rules of rendering the guaranteed amount of free medical care;

3) the list of the guaranteed amount of free medical care;

4) the list of medicines within the guaranteed amount of free medical care;

5) the scheme of placement of offices and structural divisions (the name and number by floors, indexes of traffic route to offices and departments);

6) the schedule of acceptance of doctors and work of medical and diagnostic divisions (the surname, name, middle name of the doctor completely, specialty, number of office, hours and days of acceptance is entered);
