For the purpose of ensuring execution of Items 3 of Presidential decrees of the Azerbaijan Republic "About application of the Law of the Azerbaijan Republic "About modification and amendments in the Tax code of the Azerbaijan Republic" and "About application of the Law of the Azerbaijan Republic "About the state fee" of December 26, 2001 No. 620 and 621, I order:
1. Approve the below-specified document forms (are applied):
- The declaration on the taxes collected in payment source;
- Report on deduction of the state fee.
2. Charge to the Ministry of taxes of the Nakhchivan Autonomous Republic, to provide to Department on work with large taxpayers and the companies with particular treatment, Head department of taxes of the city of Baku, territorial administrations of taxes since January 01, 2002 proper use of the forms approved by this order.
3. Charge to the Secretariat to provide (E. Gadzhiyev) the direction of copies of this order in all structural divisions of the Ministry of taxes.
4. Charge to Financial and economic management to take (R. Lyatifov) measures for providing tax authorities with forms for the form approved by this order.
5. To charge control of execution of the order to the adviser for questions of application of the tax legislation of the Ministry of taxes (V. Aliyev).
