1. Approve Regulations on licensing of veterinary activities according to appendix.

2. To the ministry of agricultural and water economy of the Republic of Uzbekistan in two weeks to approve structure and regulations on commission of experts on preparation of the conclusions about issue of licenses.

3. To impose control over the implementation of this resolution on the Secretariat of Agro-industrial complex of the Cabinet of Ministers of the Republic of Uzbekistan.

Regulations on licensing of veterinary activities

I. General provisions

1. This Provision determines procedure for licensing of veterinary activities.

2. Licensing of veterinary activities is performed by the State committee of veterinary science of the Republic of Uzbekistan (further - the licensing body).

II. Licensed requirements and conditions

7. Procedure for carrying out tender. Licensed requirements and conditions when implementing veterinary activities are below included:

compliance with law of the Republic of Uzbekistan in the field of veterinary science;

observance by the licensee of the legislation directed to prevention of damnification to the rights and legitimate interests of citizens, health, public safety when implementing activities;

realization of veterinary medicines, including medicines for the veterinary purposes, nutritional supplements and zoohygienic means, is performed by workers in staff of the subjects having the status of the legal entity with secondary special education in the direction of veterinary science and individual entrepreneurs;

availability in staff of the legal entity of the veterinary specialist with the higher or secondary vocational education and special preparation in the field of veterinary science;

availability at the individual entrepreneur of the higher or secondary vocational veterinary education and special preparation in the field of veterinary science;

advanced training of employees of the legal entity, the individual entrepreneurs performing veterinary activities, at least once in 5 years;

availability at the license applicant or the licensee of the rooms for implementation of veterinary activities belonging to it on the property right or other legal cause according to veterinary regulations and rules;

availability of the corresponding organizational specifications and the material equipment, including equipment, tools and documentation used for use of the permitted veterinary technologies;

observance of veterinary regulations and rules;

Application of the methods of diagnostics, prevention and treatment, and also veterinary medicines permitted in the Republic of Uzbekistan.

III. Documents necessary for receipt of the license, opening of branch or address modification of activities (branch)

9. Procedure for carrying out tender. For receipt of the license the license applicant submits the following documents to the licensing body:

a) the statement for licensing in which are specified:

for the legal entity - the name and form of business of the legal entity, the location (postal address), the name of organization of Bank and account number in bank institution;

for physical person - surname, name, middle name, data of the identity document of the citizen;

also phone numbers and addresses of implementation of activities (branches);
