1. This Instruction determines procedure of dispensary observation of the patients suffering from chronic or long mental disturbances (diseases) with heavy resistant or often escalating painful manifestations (further if other is not determined by this Instruction, patients), in the state psychiatric (psychoneurological) organizations of health care and other state organizations of health care which are not the psychiatric (psychoneurological) organizations of health care and rendering according to the procedure, established by the legislation of the Republic of Belarus, mental health services (further - dispensary observation).
2. Dispensary observation is performed by the specialist doctor in the field of rendering mental health services (further - the specialist doctor) by the regular surveys of the patient directed to rendering necessary mental health services to it. These surveys are performed in case of the patient's appearance in the organizations of health care specified in Item 1 of this Instruction (further - the organizations of health care), or in case of visit of the patient by the specialist doctor of the organization of health care in the place of his residence (the place of stay).
3. Dispensary observation is performed:
for patients aged up to eighteen years - in the organizations of health care rendering mental health services to the children's population in the procedure established by the legislation of the Republic of Belarus;
for full age patients - in the organizations of health care rendering mental health services to adult population in the procedure established by the legislation of the Republic of Belarus.
4. Establishment of dispensary observation of the patient is performed based on the conclusion of the medical and consulting commission of the organization of health care (further - VKK).
Information on establishment (termination) of dispensary observation of the patient is entered in the medical record of the outpatient (further - the medical record). The face of the medical record is marked by one of the following methods:
"DN" - dispensary observation;
"SU" - subgroup of dispensary observation of the patient with socially dangerous tendencies and actions ("Special accounting");
"OU" - subgroup of dispensary observation of the patient with suicide signs ("Special accounting");
"PL" - subgroup of dispensary observation of the patient concerning whom determination (resolution) of court on application of enforcement powers of safety and treatment ("Forced treatment") is taken out.
Besides, concerning the patient for whom dispensary observation is established the control card of dispensary observation on which face the corresponding marking is also applied is drawn up ("DN", "SU", "OU", "PL").
5. In case of establishment of dispensary observation the specialist doctor tells the patient or his legal representative in form, available to understanding, taking into account condition of mental health of the patient information on establishment of dispensary observation, nature of mental disturbance (disease), the purposes, methods, durations of dispensary observation and recommended treatment, the expected results and consequences of refusal of dispensary observation and (or) treatment of the patient.
The fact of reporting of information, specified in part one of this Item, is drawn up by entry in the medical record.
6. Diagnostics of condition of mental health of patients for whom dispensary observation is established is performed taking into account the International Statistical Classification of the diseases and problems connected with health.
Treatment of patients for whom dispensary observation is established is carried out according to clinical protocols of delivery of health care to patients with mental and behavioural disturbances.
7. When implementing dispensary observation uniform groups (subgroups) of patients united by specifics and expressiveness of psychopathological symptomatology, nature and extent of social disadaptation and the need for the relevant medical and rehabilitation activities are created (further if other is not determined by this Instruction, - groups (subgroup) of dispensary observation).
8. Frequency of surveys of the patient for whom dispensary observation is established is determined by the specialist doctor taking into account condition of mental health of the patient, the carried-out treatment, according to group (subgroup) of dispensary observation.
9. Need of the translation of patients concerning whom dispensary observation is established from one group (subgroup) of dispensary observation in other group (subgroup) of dispensary observation is determined by dynamics of condition of their mental health, its stability and extent of social and labor adaptation of patients. The decision on such translation is made by the specialist doctor.
10. The first group of dispensary observation is constituted by patients:
written out from psychiatric hospital;
with temporary disability (capability to training) owing to mental disturbance (disease) and needing active therapy;
with aggravations of mental disturbance (disease), needing active therapy and intensive observation of the specialist doctor;
with aggravations of mental disturbance (disease), needing rendering planned medical care in stationary conditions.
