ORDER OF FUND OF THE STATE-OWNED PROPERTY OF UKRAINE

of May 29, 2012 No. 754

About approval of the Procedure for creation and representation of requests for public information which manager is the Fund of state-owned property of Ukraine, and forms for representation of request for receipt of public information

According to the Law of Ukraine "About access to public information", the Presidential decree of Ukraine of 05.05.2011 No. 547 "Questions of ensuring with executive bodies of access to public information" and item 4 of the resolution of the Cabinet of Ministers of Ukraine of 25.05.2011 No. 583 "Questions of accomplishment of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies" PRIKAZYVAYU:

1. Approve:

1.1. Procedure for creation and representation of requests for public information which manager is the Fund of state-owned property of Ukraine which is applied.

1.2. Forms for representation of request for receipts of public information which are applied.

2. Determine Management of interaction with the Verkhovna Rada of Ukraine and communication providing Department of management of state-owned property, contractual management and communication providing with special structural division which will organize in accordance with the established procedure access to public information which manager is the Fund of state-owned property of Ukraine.

3. To management of interaction with the Verkhovna Rada of Ukraine and communication providing Department of management of state-owned property, contractual management and communication providing to provide this order in accordance with the established procedure on state registration to the Ministry of Justice of Ukraine and to provide its placement on the official website of Fund of state-owned property of Ukraine.

4. Confer the personal responsibility for accomplishment of the Law of Ukraine "About access to public information", the Presidential decree of Ukraine of 05.05.2011 No. 547 "Questions of ensuring with executive bodies of access to public information", this order, and also for reliability and timely provision of replies to the requests on public information on heads of structural divisions of Fund of state-owned property of Ukraine.

5. To regional departments and representative offices of Fund of state-owned property of Ukraine to take measures concerning development and entry into force of the Procedure for creation and representation of requests for public information which manager are regional departments, representative offices of Fund of state-owned property of Ukraine.

6. This order becomes effective from the date of its official publication.

7. To impose control over the implementation of this order on the authorized officer of Fund of state-owned property of Ukraine according to distribution of functional obligations.

Procedure for creation and representation of requests for public information which manager is the Fund of state-owned property of Ukraine

І. Creation and representation of requests for public information

1.1. This Procedure determines the mechanism of ensuring state-owned property of Ukraine with Fund (further - Fund) operational consideration and provision of answers to physical persons, legal entities, consolidation of citizens without the status of the legal entity, except subjects of powers of authority (further - Requesting), on requests for public information which manager is the Fund of state-owned property of Ukraine.

1.2. In this Procedure terms are used in the value given in the Law of Ukraine "About access to public information" (further - the Law).

1.3. Access to public information of Fund is provided by provision of information on requests for public information.

1.4. The relations in the field of addresses of citizens are governed by the Law of Ukraine "About the address of citizens".

1.5. The request for receipt of public information is sent in Fund in oral or written form during personal acceptance or departure siding by mail, to the telefax, e-mail or phone at choice of requesting.

1.6. The written request on receipt of public information is sent in any form.

1.7. The request for information shall contain:

surname, name, middle name (name) requesting, the postal address or the e-mail address, and also number of the means of communication (if it is);

the general description of information or type, the name, details or contents of the document concerning which the request is made (if requesting it it is known);

the signature and date (on condition of submission of the written request).

1.8. For the purpose of simplification of the procedure of execution of the written request requesting can use forms for representation of request for the receipts of public information approved by this order.

1.9. Forms of requests are posted on the official website of Fund.

1.10. Requesting can fill request directly on the official website of Fund according to the specified form.

1.11. During representation of request for information requesting specifies form of receipt of information, convenient for it.
