According to Items 7, of 11, of the 12th resolution of the Cabinet of Ministers of Ukraine of April 13, 2011 "Some questions of issue of permission to use of the objects and rooms intended for carrying out the activities connected with the address of drugs, psychotropic substances and precursors" I ORDER to No. 469:
1. Approve:
1.1. Form of the act of inspection of the objects or rooms intended for carrying out the activities connected with development, production, production, storage, transportation, acquisition, realization (leave), use and destruction of drugs, psychotropic substances and precursors which is applied.
1.3. Form of the magazine of registration of acts of inspection of the objects or rooms intended for carrying out the activities connected with the address of drugs, psychotropic substances and precursors which is applied.
2. To provide to chiefs of head departments, Departments of MIA of Ukraine in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol documentary registration of inspection of the objects and rooms intended for carrying out the activities connected with the address of drugs, psychotropic substances and precursors.
3. (Demidov O. M.) to provide to management of fight against the illegal address of drugs submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.
4. To impose control over the implementation of the order on the First Deputy Minister of the militia lieutenant general of Black S. P.
5. Send this order on accessory.
6. This order becomes effective from the date of its official publication.
