According to part six of Article 257 of the Customs code of Ukraine the Cabinet of Ministers of Ukraine decides:

1. Approve form of the customs declaration for written declaration of goods which move through customs border of Ukraine citizens for the personal, family and other needs which are not connected with conducting business activity (further - the customs declaration) which is applied.

2. Determine that customs declaration forms can be produced by citizens by printout using means of the computer equipment or their copying on paper of the A4 format (210 x 296 millimeters) in the form approved by this resolution.

3. Provide to the State Customs Service in three-months time production of customs declaration forms in the form approved by this resolution.

4. To the Ministry of Finance to develop and approve procedure for filling of the customs declaration in three-months time.

5. To the State Customs Service to perform till October 1, 2012 customs clearance of goods which move through customs border of Ukraine citizens for the personal, family and other needs which are not connected with carrying out business activity, using the forms made in form of the written customs declaration for the announcement by citizens of data on objects which move them through customs border of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of July 15, 1997 No. 748 "About the list of data which appear citizens in the established form in case of movement by them through customs border of Ukraine of objects" (The Official Bulletin of Ukraine, 1997, number 29, of page 33).

6. Recognize invalid resolutions of the Cabinet of Ministers of Ukraine according to the list which is attached.

7. This resolution becomes effective since June 1, 2012.
