About approval of document forms concerning adoption
According to Items 12, 23, 27, 102, 104 Procedures for carrying out activities for adoption and implementation of the supervision of observance of the rights of the adopted children approved by the resolution of the Cabinet of Ministers of Ukraine of October 08, 2008 No. 905 (with changes), and for the purpose of methodical ensuring activities of services for children of PRIKAZYVAYU:
1. Approve document forms concerning adoption which are attached.
2. Approve the certificate of passing of course of training concerning education of the orphan children and children deprived of parent guardianship which is applied.
3. Approve the certificate of acceptance of all exhaustive measures for the child's device in seven citizens of Ukraine which is applied.
4. Approve Requirements to execution of the conclusion about feasibility of adoption and its compliance to interests of the child which are applied
5. Approve form of the electronic register of the children adopted by the citizens of Ukraine living beyond its limits, and foreigners which is applied.
6. Approve forms of statements for adoption cancellation which are applied.
7. Declare invalid the order of the Ministry of Ukraine for family, youth and sport of December 02, 2010 No. 4315 "About approval of document forms concerning adoption", registered in the Ministry of Justice of Ukraine on December 21, 2010 for No. 1300/18595.
8. To department on adoption and protection of the rights of the child (Filipishina A. A.) submit this order in the procedure established by the legislation on state registration to the Ministry of Justice of Ukraine.
9. This order becomes effective from the date of its official publication.
10. To impose control over the implementation of this order on the deputy minister L. Drozdova.

The conclusion about feasibility of adoption and its compliance to interests of the child shall contain the following information:
1. Date and the purpose of the appeal of applicants to service for children.
2. Surname, name, middle name, passport data, the residence (the country of accommodation), information on employment of persons who wish to adopt the child.
3. Data on passing of preparation concerning education of orphan children, the children deprived of parent guardianship with the reference to the document certifying such preparation and recommendations concerning adoption (age, sex, the number of children, condition of their health).
4. Data on capture on accounting of candidates for adoptive parents, availability of the conclusion about opportunity to be adoptive parents.
5. Data on stay in scrap, availability or lack of criminal record, state of health of each of applicants, material and financial provision of family.
6. Motives of adoption, availability of the (adopted) children, line item concerning their education (in case of availability of the adopted children information concerning condition of their health, development, adaptation in family, results of implementation of supervision of observance of their rights is specified).
7. Conditions of accommodation of family, availability or possibility of creation for the adopted child of the certain room or the vacation spot (in case of accommodation of own children or other family members together with applicants their relation to adoption is specified).
8. Surname, name, middle name, birth date, the residence (stay) of the child who is wished to be adopted.
9. Legal basis for adoption of the child.
10. Circumstances of acquaintance of candidates for adoptive parents to the child and establishments of contact with it.
