ORDER OF THE MINISTRY OF FINANCE OF UKRAINE

of February 22, 2012 No. 246

About approval of samples of forms of the single administrative document, additional sheets, amendments and specification and their Technical description

1. Approve samples of forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form and the specification of the MD-form 8, which are applied.

2. Approve the Technical description on forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-form 6, on the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form and the specification of the MD-form 8, which are attached.

4. (Chmeruk M. O.) and to Department of the organization of customs control and registration of the State Customs Service of Ukraine (Syomk S. M.) in accordance with the established procedure to provide to department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

5. This order becomes effective in 10 days from the date of its official publication.

6. To impose control over the implementation of this order on the First Deputy Minister of Finance of Ukraine Myarkovsky A. I. and First Deputy Chairman of the State Customs Service of Ukraine of Dorokhovsky O. M.

Technical description on forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD5, form of amendment to it to the cargo customs declaration of the MD-7 form and specification of the MD-8 form

1. Forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form and the specification of the MD-8 form are produced on self-transfer paper which has density at least than 40 grams on square meter.

Data which contain in graphs of the cargo customs declaration are reproduced by self-copying on copies of forms of the single administrative document of the MD-2, MD-4 forms and additional sheets of the MD-3, MD-5 forms to them according to appendices 1, 2 to this Technical description. In cases when data do not be copied from one copy on another, deactivation of sensitivity of self-transfer paper is applied.

Detachable part of forms answers the A4 format. Binding of forms is performed by punched method. Perforation large.

2. Color of the text and lines of forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6 form and the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form - green.

Color of the text and lines of the form of the specification of the MD-8 form - black.

3. The applied font of font - Arial.

Drawing and size of forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form:

the name and sequence number of the column A - 5.5 pt Regular (capital letters);

names and sequence numbers of columns B, C, D, E, F, G, H, I, J, D/J, E/J, 376 pt Regular (capital letters);

the name and sequence number of column 1 - 6 pt Bold (capital letters);

basic data (the name of the company - producer of forms) are put down in the bottom right corner of forms - 4 pt Regular;

other texts - 6 pt Regular.

Drawing and size of the form of the specification of the MD-8 form:

the texts "UKRA¶NA", "SPETSIFIKATS_YA" - 10 pt Bold (capital letters); text "MD-8 Form" - 8 pt Regular;

text "(P_dpis)" - 6 pt Regular;

"No." texts, "The description and goods quantity", '"Commodity code", "Gross weight, kg", "Invoice cost", "Customs value" - 8 pt Bold.

4. Color marking of forms of the single administrative document of the MD-2 form and additional leaf of the MD-3, form of amendment to it to the cargo customs declaration of the MD-6, form of the single administrative document of the MD-4 form and additional leaf of the MD-5, form of amendment to it to the cargo customs declaration of the MD-7 form:

1) on forms of the single administrative document of the MD-2 form and additional leaf to it of the MD-3 form and amendment to the cargo customs declaration of the MD-6 form copies 1/6, 2/7, 3/8 and 4/5 shall have at the right edge the continuous field and on the right from it the dotted field of red, green, yellow and blue colors respectively;

2) on forms of the single administrative document of the MD-4 form and additional leaf to it of the MD-5 form and amendment to the cargo customs declaration of the MD-7 form:

copies 1, of 2, of 3, 5 shall have at the right edge the continuous field of red, green, yellow and blue colors respectively; copies 4, of 6, 7 and 8 shall have at the right edge the dotted field of blue, red, green and yellow colors respectively;

3) on copies of 1/6, 2/7 and 4/5 forms of the single administrative document of the MD-2 form and additional leaf to it of the MD-3 form and on copies 1, of 4, of 5, 7 forms of the single administrative document of the MD-4 form and additional leaf of the MD-5 form of column 1 to it (except for average subgraphs), 2, 3, 4, 5, 6, 8, 15, 17, 18, 19, 21, 25, 27, 31, 32, 33 (the first subsection at the left), 35, 38, 40, 44, 50, 51, 52, 53, 55 and 56 shall have light green background.

5. Thickness of the applied lines:

fat lines - 2 pt;

thin lines - 1 pt;

dashed lines - 1 pt.
