1. These rules of rendering palliative care and sisterly leaving (further - Rules) are developed according to Item 3 of article 53 of the Code of the Republic of Kazakhstan of September 18, 2009 "About health of the people and health care system" and determine procedure for rendering palliative care and sisterly leaving by the medical organizations, irrespective of pattern of ownership.
2. In these rules the following concepts are used:
1) palliative care - the direction of medical and social activities which purpose is the improvement of quality of life and general condition of patients with incurable diseases in terminal (final) stage confirmed with the medical certificate issued by the organization of health care;
2) sisterly leaving - the direction of medical and social activities, on rendering the psychological and public assistance and household leaving to persons with serious illness, confirmed with the medical certificate, the issued organization of health care;
3) the portal of Bureau of hospitalization (further - the Portal) - single system of electronic registration, accounting, processing and storage of the directions of patients for planned hospitalization in hospital within the guaranteed amount of free medical care;
4) the stationary help - form of provision of qualified, specialized and highly specialized medical care with the round-the-clock medical observation;
5) the hospital-replacing help - form of provision of pre-medical, qualified, specialized and highly specialized medical care with medical observation lasting from four till eight o'clock during the day.
3. Palliative care is given under the leadership of the doctor to incurable patients in terminal (final) stage of disease in specialized structural divisions, the independent medical organizations (hospices) or in the form of hospital at home.
4. Sisterly leaving is performed in the cases which are not requiring medical observation in specialized structural divisions, the independent medical organizations (the organization of sisterly leaving) or in the form of hospital at home.
5. The services in rendering palliative care and sisterly leaving which are not entering the list of the guaranteed amount of free medical care (further - GOBMP), appear on paid basis according to the procedure, determined by the Government of the Republic of Kazakhstan.
6. Indications for hospitalization of patients in the organization of palliative care and sisterly leaving are:
1) diseases or conditions according to the list of categories of the population, subjects of palliative care and to the sisterly leaving approved by the order of the Government of the Republic of Kazakhstan;
2) need of matching of the supporting, anesthetizing therapy for the subsequent transfer of the patient to treatment in out-patient conditions;
3) availability of social and psychological indications: condition of depression, jet condition or conflict situation in family, lack of living conditions for care of the patient.
7. Palliative care and sisterly leaving by the patient appear in the form of the stationary and hospital-replacing help.
8. Hospitalization of the patient in hospital within GOBMP is performed:
1) in planned procedure in the direction of specialists of primary health care (further - PHC) or the profile specialist of the medical organization through the portal with informing the patient or his legal representative on date of hospitalization in hospital;
2) according to the emergency indications regardless of direction availability.
9. The hospital-replacing help in form of palliative care and sisterly leaving of the help is performed in hospitals in the form of qualified medical care and sisterly leaving lasting from four till eight o'clock during the day at home.
10. In case of treatment of the patient:
1) in day hospital the map of sick day hospital in the form approved by authorized body in the field of health care is filled in (further - authorized body);
2) in hospital the map of sick hospital at home in the form approved by authorized body is filled in at home.
11. The information about the patient is registered in primary accounting medical documentation in:
1) register of patients of day hospital (polyclinic) in the form approved by authorized body;
2) register of patients of hospital at home in the form approved by authorized body.
12. When rendering palliative care to the patient the doctor of day hospital (polyclinic) and hospital at home, performs survey of the patient and carries out treatment adjustment daily.
13. When rendering sisterly leaving to the patient the average health worker of day hospital (polyclinic) and hospital performs leaving daily at home.
14. Palliative care in the form of psychological and moral support is performed at early stage of any chronic, incurable disease in parallel with other types of treatment.
15. In case of deterioration in condition of the patient who is at home where the round-the-clock observation is required hospitalization in hospital is performed without delay.
16. Treatment duration from the moment of arrival of the patient constitutes:
1) in day hospital at least 8 working days;
2) in hospital at home at least 8 working days.
17. In case of the statement of the patient who received the hospital-replacing help the statistical map in the form approved by authorized body is filled in.
18. The statement from the medical record of the patient with necessary recommendations is handed out to the patient or his legal representative under list and transferred to the PSMP organization for the place of attachment.
