According to the Law of Ukraine "About procedure for carrying out reforming of health care system in the Vinnytsia, Dnipropetrovsk, Donetsk regions and the city of Kiev" and on accomplishment of the order of the Cabinet of Ministers of Ukraine of 16.08.2011 No. 37565/1/-11 of PRIKAZYVAYU:

1. Approve:

1.1. Procedure for the organization of medical attendance and the direction of patients in health care institutions which render secondary (specialized) and tertiary (highly specialized) medical care which is applied.

1.2. Procedure for placement and calculation of quantity of crews of emergency medical service which is applied.

2. To chiefs of managements of health care Vinnytsia, Dnipropetrovsk, Donetsk regional and Kiev city public administrations to provide accomplishment of this order.

3. (Koblosh V. V.) to provide to department of normative legal support submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

4. This order becomes effective from the date of its official publication.

Procedure for the organization of medical attendance and the direction of patients in health care institutions which render secondary (specialized) and tertiary (highly specialized) medical care

I. General provisions

1.1. This Procedure regulates the basic principles of the organization of provision of medical care in institutions VMP and TMP and conditions under which the patient goes to institutions of health care of the state and municipal patterns of ownership which provide secondary (specialized) medical care (further - VMP) and tertiary (highly specialized) medical care (further - TMP), and also ways of arrival of patients to the specified institutions.

1.2. Provision of medical care in institutions VMP and TMP is performed in stationary and out-patient conditions.

1.3. VMP is provided:

in stationary conditions - multi-profile hospitals of intensive treatment (further - BIL) the first and second levels, multi-profile children's hospitals (clinical hospitals) of intensive treatment (further - DETBIL), hospitals of recovery treatment (further - BVL), hospitals of planned treatment (dalshebpl), the hospices specialized by the medical centers (further - SMTs);

in out-patient conditions - divisions of the out-patient sector of hospitals of different type, the centers of medical consultations and diagnostics (the konsultativnodiagnostichesky centers) (further - KDTs).

1.4. TMP is provided in stationary and out-patient conditions by multi-profile regional hospitals (clinical hospital), multi-profile regional children's hospitals (clinical hospitals), the highly specialized medical centers (in the directions).

1.5. The out-patient help in institutions VMP and TMP can directly be provided in institution or remotely by carrying out telemedicine consultations.

1.6. The direction of patients in institutions VMP and TMP is performed in planned and emergency procedure.

1.7. The direction of the patient in planned procedure provides preliminary coordination of date and time of carrying out inspection, consultation or hospitalization which is performed by the attending physician in institution VMP or TMP.

The maximum agreed time frame of carrying out inspection, consultation or hospitalization cannot exceed one month from the date of filling of the direction on inspection, consultation or hospitalization in planned procedure, and for oncological patients - ten days.

1.8. The direction of the patient in the emergency procedure in institution VMP or TMP is performed without preliminary coordination of date and time of hospitalization (consultation).

1.9. The independent address of the patient - the address of the patient to institution VMP or TMP without the direction.

1.10. Planned hospitalization (consultation) of the patient - hospitalization (consultation) in the direction of the doctor which is performed in the presence of indications for such hospitalization according to earlier approved date and hospitalization time.

1.11. Hospitalization (consultation) of the patient on emergency pokazaniyamgospitalization (consultation) of the patient in the presence of indications for such hospitalization (consultation) which is performed round the clock without preliminary coordination of date and time of hospitalization (consultation) irrespective of the place of residence (stay) of the patient.

Hospitalization according to the emergency indications is performed round the clock.

1.12. The direction on hospitalization (consultation) provides filling with the doctor or health worker of service of emergency medical service of the corresponding direction on hospitalization (consultation) in institution VMP or TMP.

1.13. Absence at the patient of identity documents, cannot be cause of failure in the direction on consultation or hospitalization and implementation of hospitalization or consultation in the emergency procedure in the presence of the corresponding indications.

II. Procedure for the direction of patients in institutions of secondary (specialized) medical care

2.1. The directions in institution VMP in planned procedure can perform:

doctors of general practice are family doctors (further - doctors of OP-SV), therapists district police officers and pediatricians district police officers of the centers of primary health care (further - TsPMSD);

doctors of OP-SV who carry out economic activity on medical practice as physical persons - the entrepreneurs consisting with TsPMSD in the civil relations;

specialists doctors of institutions VMP of other specialization and TMP.

2.2. The directions of the patient in institution VMP or TMP in the emergency procedure can perform:

doctors of OP-SV, therapists district police officers and pediatricians local TsPMSD;

doctors of OP-SV who carry out economic activity on medical practice as physical persons - the entrepreneurs consisting with TsPMSD in the civil relations;

specialists doctors of TsPMSD;

specialists doctors of institutions VMP and TMP;

service of emergency medical service.
