According to Item 1 of article 15 of the Law of the Republic of Kazakhstan of January 6, 2011 "About the state control and supervision in the Republic of Kazakhstan" PRIKAZYVAYEM:

1. Approve forms of checking sheets in the sphere of private entrepreneurship:

1) behind holding procedure of external observation according to appendix 1 to this order;

2) behind holding rehabilitation procedure according to appendix 2 to this order;

3) behind carrying out bankruptcy proceedings according to appendix 3 to this order.

2. To provide to committee on work with poor debtors of the Ministry of Finance of the Republic of Kazakhstan (Usenov N. D.):

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) official publication of this order after its state registration in the Ministry of Justice of the Republic of Kazakhstan;

3) publication of this order on official Internet resource of the Ministry of Finance of the Republic of Kazakhstan.

3. To impose control of execution of this order on the Vice-Minister of Finance Shukputov A. M.

4. This order becomes effective after ten calendar days after day of its first official publication.
