About the organization of licensing of separate types of activity

According to the Federal Law "About Licensing of Separate Types of Activity" Government of the Russian Federation decides:

1. Approve the enclosed list of the federal executive bodies and the state corporations performing licensing of specific types of activity.

2. Determine that:

a) The Federal Service on supervision in the field of health care:

is the federal executive body exercising control and supervision of completeness and quality of implementation by public authorities of subjects of the Russian Federation of the powers specified regarding 1 article 15 of the Federal law "About Bases of Protection of Public Health in the Russian Federation", with the right of the direction of instructions about elimination of the revealed violations and about accountability of officials, acting as on implementation of delegated powers;

keeps in electronic form the unified register of licenses, including the licenses provided by public authorities of subjects of the Russian Federation;

provides observance by licensees of licensed requirements by means of implementation of the state control (supervision) (except for the licensees who submitted applications for modification of the register of licenses), performs powers on suspension, renewal of action and cancellation of licenses, representation in the procedure for carrying out monitoring and efficiency evaluation of licensing of specific types of activity of information established by the Government of the Russian Federation for the purpose of carrying out monitoring and efficiency evaluation of licensing of the types of activity specified in Item 1 of part 1 of article 15 of the Federal law "About Bases of Protection of Public Health in the Russian Federation";

sends in electronic form by means of single system of interdepartmental electronic interaction within 5 working days to the public authorities of subjects of the Russian Federation performing powers, specified in Item 1 of part 1 of article 15 of the Federal law "About Bases of Protection of Public Health in the Russian Federation", verified copies of orders on suspension and renewal of action of licenses, purpose of checks of licensees, the copy of acts of checks of licensees, instructions about elimination of the revealed violations of licensed requirements, protocols on administrative offenses, resolutions on purpose of the administrative punishments and other documents connected with implementation of checks of observance by licensees of the licensed requirements (except for the licensees who submitted applications for modification of the register of licenses) performing types of activity parts 1 of article 15 of the Federal law "About Bases of Protection of Public Health in the Russian Federation" specified in Item 1, for inclusion in licensed cases;

b) The Federal Service for Supervision in Education and Science is the federal executive body exercising control of realization of the government of subjects of the Russian Federation of the powers of the Russian Federation by executive bodies in the field of licensing of educational activities transferred according to article 7 of the Federal law "About Education in the Russian Federation" is operator of information system in which executive bodies of the government of subjects of the Russian Federation perform maintaining registers of licenses.

3. Activities, the license for which implementation is provided by the licensing body of one subject of the Russian Federation, can be performed in the territories of other subjects of the Russian Federation on condition of the prior notice in writing by the licensee of licensing authorities of subjects of the Russian Federation in the territory of which the licensee intends to perform the licensed type of activity, about such intention. In the notification the following data are specified:

complete and (if is available) the abbreviated name, including trade name, and form of business of the legal entity, the address of its location, the address of places of implementation of the licensed type of activity which intends to perform the licensee, and (or) other data allowing to identify the place of implementation of the licensed type of activity which are specified in need of amendment to the postal address or instead of it in case of its absence, and also phone numbers and e-mail addresses of the legal entity;

surname, name and (if is available) middle name of the individual entrepreneur, the address of its residence, the address of places of implementation of the licensed type of activity which intends to perform the licensee, and (or) other data allowing to identify the place of implementation of the licensed type of activity which are specified in need of amendment to the postal address or instead of it in case of its absence, data of the document proving his identity and also phone numbers and e-mail addresses of the individual entrepreneur;

data on date of provision of the license and its registration number;

the licensed type of activity according to part 1 of article 12 of the Federal law "About Licensing of Separate Types of Activity" which the licensee intends to perform, with indication of the performed works and the rendered services constituting the licensed type of activity;

the data confirming compliance of the licensee to licensed requirements in case of performance of works, rendering the services constituting the licensed type of activity which the licensee intends to perform in the new place, according to parts 7 and 9 of article 18 of the Federal law "About Licensing of Separate Types of Activity";

date of the expected beginning of implementation of the licensed type of activity in the territory of the subject of the Russian Federation.

The statement for modification of the register of licenses is attached to the notification.

The licensee has the right to provide on own initiative together with the statement for modification of the register of licenses the copy of the document confirming payment of the state fee for modification of the register of licenses based on the statement for modification of the register of licenses. In case of its non-presentation the authorized body of the executive authority of the subject of the Russian Federation requests the corresponding data according to the procedure of interdepartmental information exchange.
