Requirements to the list and contents of documents for provision of the expert opinion on the level of emergency situation of technogenic and natural nature

I. General provisions

1.2. These Requirements determine the list and contents of the documents necessary for consideration and preparation of the Ministry of Emergency Situations of Ukraine of the expert opinion on the level of emergency situation of technogenic or natural nature (daleeekspertny conclusion).

The expert opinion prepares based on information analysis which the documents determined by these Requirements and comparisons of the obtained data with values of criteria of levels of the emergency situations determined by Procedure for classification contain.

II. List and contents of the documents necessary for preparation of the Expert opinion

2.1. The list of documents which are filed by the central executive bodies, Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city public administrations to the Ministry of Emergency Situations of Ukraine for preparation of the Expert opinion:

the appeal of the central executive body, Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city public administrations (further - the address);

the minutes of regional commission concerning technogenic ecological safety and emergency situations (further - the protocol) rather preliminary classification of emergency situation by the form, to classification signs and level;

calculation of the loss caused as a result of emergency situation;

certificate of violation of normal conditions of life activity;

certificate of hydroweather conditions.

In the list of documents which are filed by the central executive bodies and are used by Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city public administrations for preparation of the Expert opinion, the Ministry of Emergency Situations of Ukraine attaches messages on origin or threat of emergence of emergency situation and liquidation of its effects (further - the message) which is provided by territorial authorities of department of the Ministry of Emergency Measures of Ukraine.

2.2. Documents which are filed for preparation of the Expert opinion shall meet the requirements of the Approximate instruction on clerical work in the ministries, other central executive bodies, Council of Ministers of the Autonomous Republic of Crimea, local executive bodies approved by the resolution of the Cabinet of Ministers of Ukraine of 17.10.97 No. 1153.

2.3. Contents of documents which are filed to the Ministry of Emergency Situations of Ukraine for preparation of the Expert opinion:

2.3.1. The short description of emergency situation in which the reasons of its origin and effect are displayed is provided in the address; the list of settlements and hour interval of action of emergency situation, the number of the dead and affected population, term of violation of normal conditions of life activity of the population is specified (the number of persons is specified); the amount of financing (from local budgets, target regional and budget programs, subventions) the actions directed to prevention of emergence of emergency situation, and also amount of the assignments directed to mitigation of consequences of emergency situation at the expense of means of the companies, organizations and the organizations in the territory of which it arose, and in addition - at the expense of reserve funds of local budgets and material reserves (according to Procedure for creation and use of material reserves for prevention, the liquidations of emergency situations of technogenic and natural nature and their effects approved by the resolution of the Cabinet of Ministers of Ukraine, of 29.03.2001 No. 308).

the list of citizens who as a result of emergency situation stayed at home without apartment (rooms) and/or evacuated - surnames, names and middle names of citizens are entered; birth dates; name and details of identity documents; registration dates to addresses of the lost residences (registration), the sheet (the area, property rights, residual value, and so forth) bureau of technical inventory count, rather apartment houses (rooms);
