For the purpose of ensuring execution of Items 1. 9, and 1.10 Presidential decrees of the Azerbaijan Republic of July 2, 2010 No. 289 "About compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases" the Cabinet of Ministers of the Azerbaijan Republic decides:

1. Approve "The list of documents for purpose of insurance payment for compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases" and "Rules of appointment, suspension and renewal of issue of insurance payment for compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases" (are applied).

2. Determine that governed, issue conditions, and the benefits amounts performed to the worker whose health is broken owing to production accident or occupational disease or to family members and other faces of the worker who died for this reason before the date of entry into force of the Law of the Azerbaijan Republic "About compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases" which are on care are regulated by the resolution of the Cabinet of Ministers of the Azerbaijan Republic "About approval of the Rules, conditions of issue and benefits amount performed to the worker whose health is broken owing to production accident or occupational disease, or to family members and other faces of the worker who died for this reason" of January 3, 2003 No. 3 which are on care (Collection of the legislation of the Azerbaijan Republic 2003 No. 1, Article 66; 2004 No. 8, Article 657; 2008 No. 2, Article 130, No. 9, Article 855; 2009 No. 10, Article 857; 2010 No. 3, Article 262).

The list of documents for purpose of insurance payment for compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases

1. This List is prepared for the purpose of ensuring execution of Item 1.9 of the Presidential decree of the Azerbaijan Republic of July 2, 2010 No. 289 "About compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases" and establishment by the insurer of insurance payment for compulsory insurance from loss of professional working capacity as a result of labor accidents and occupational diseases is performed on the basis of the following documents:

1.1. on monthly or lump insurance payment:

1.1.1. Ceased to be valid according to the Resolution of the Cabinet of Ministers of the Azerbaijan Republic of 15.05.2023 No. 156

1.1.2. the insurance claim signed by other person receiving payment in case of death insured as a result of insured event or total loss of physical and (or) intellectual capacities;

1.1.3. the copy of the statement of labor accident which is drawn up in the form established by the legislation or the judgments about incident with insured labor accident;

1.1.4. in case of loss insured professional working capacity as a result of accident, the copy of the decision issued by Medico-social Commission of experts, confirming extent of loss insured professional working capacity as a result of labor accident and occupational disease;

1.1.5. in case of death insured as a result of insured event or case of violation of health which reason the specified case, the copy of the certificate of his death directly is;

1.1.6. in case of death insured as a result of insured event or case of violation of health which reason the specified case, the reference on the family members who are on its care and also the certificate of the fact that one of not working parents - the wife (husband) or other family member, regardless of working capacity, cares for the children, grandsons, brothers and sisters who were on care insured and did not reach fourteen years or reached, however, according to medical institution, needing for health reasons guardianship of other person directly is;

1.1.6-1. if insured, receiving monthly insurance indemnity in connection with loss of professional working capacity as a result of insured event, will die because of the violation in the state of health caused by directly this case, the solution of medico-social commission of experts confirming communication of death with insured event;

1.1.7. in case of loss insured professional working capacity as a result of insured event, the copy of the identity certificate, and in case of his death receiving payment of other person (persons).

1.1.7-1. in case of death insured as a result of insured event if the members of his family who are considered as beneficiaries are his parents, the copy of its certificate of birth if children, their certificates of birth if the husband (wife) - marriage certficates;

1.2. In case of purpose of additional insurance payment, in addition to the relevant documents determined by Item 1.1. this List, the copy of the solution of Medico-insurance Commission of experts concerning the fact what insured needs this help and types of care and that the right to receive them is free of charge stipulated by the legislation is attached.

1.3. Purpose of insurance payments for the annuitentny agreement is performed and insurance payment is paid to the beneficiary from the date of, approved in the specified agreement under this agreement.

2. The insurer who obtained information on incident of insured event shall provide person receiving payment, the questionnaire on insurance claim for representation of the insurance claim provided in Item 1.1.2 of this List and if it is necessary, to help it with filling of this leaf.

3. Person receiving payment shall submit to the insurer the documents determined by Items 1.1. 2, 1.1.5 and 1.1.7 these Lists. In case other documents are not provided by person receiving payment, they shall be received by the insurer by the official request (requests) made within 3 working days from the date of receipt of insurance claim.
